【摘要】 目的观察局部载体抗生素治疗四肢创伤后感染的疗效。方法26例四肢创伤后感染患者采用局部彻底清创后植入抗生素载体。无明显骨缺损者选用庆大霉素珠链,有骨缺损者采用妥布霉素硫酸钙颗粒；去除内固定后骨折不稳定者改用单侧外固定支架固定；有软组织缺损者予皮瓣转移修复。结果所有患者平均随访(10．82±9．76)个月。创口一期愈合,无分泌物流出,肢体红肿、热痛等症状消失。术后4w骨折周围有不同程度的新骨痂生成,术后12个月除1例肱骨骨折、1例胫骨骨折、1例尺骨骨折不愈合外,余骨折均愈合,肢体功能基本恢复正常。3例骨折不愈合者予自体髂骨植骨术,其中2例骨折已愈合,1例骨折正在愈合中。结论清创加局部栽体抗生素的应用能够有效治疗四肢创伤后感染。更多还原

【Abstract】 Objective To observe the effect of carrier antibiotics in the treatment of infection after limb tru- ma.Methods From March 2001 to February 2005,we treated 26 cases of infection after trauma by using thorough de- bridement and carrier antibiotics implantation.Septopal was used in those who had no bone defect,Osteoset-T was chose in the others.Singeral lateral brace was fixed in unstable fracture when interfixation was broken away.Skin defects after infection were covered with different kinds of flaps.Results All patients were followed up average of 10.8 months.In- fection was controlled,wound healed,all symptoms of inflammation disappeared.New bone could be found in 4 weeks after operation.All fracture were union and limbs’ function recovered 12 months after operation except 1 humerus frac- ture,1 tibia fracture and 1 ulnar fracture.Autogenous ilium grafts were performed in the 3 cases of exception which 2 cases healed and 1 case was in the healing process.Conclusion It is effective to control infection after trauma by using thorough debridement and carrier antibiotics implantation.更多还原
