多层螺旋CT颅外段颈动脉血管造影技术分析
Technical analysis of extracranial carotid artery with multiple detector-row spiral CT
【摘要】 目的探讨多层螺旋CT颈动脉血管造影的最佳扫描延迟时间和图像后处理技术的应用。方法采用GE公司L ight Speed P lus/4 i多层螺旋CT扫描仪对156例患者颈动脉进行容积扫描,分两组,第1组72例利用小剂量试验扫描确定延迟时间;第2组84例常规延迟16~20 s。在ADW 4.0工作站分别进行二维多平面重建(MPR)、曲面重建(CPR)、智能血管分析及三维表面遮盖法(SSD)、容积再现技术(VR)、最大密度投影(M IP)、仿真内窥镜(VE)等图像后处理。结果156例患者中152例扫描成功,4例扫描失败,第1组1例患者不能配合,第2组1例延迟时间短,2例延迟时间偏长。颈动脉体瘤3例,颈动脉瘤4例,假性动脉瘤1例;余144例288支颈动脉,正常176支、轻度狭窄72支、中度狭窄21支、重度狭窄15支、完全闭塞4支。结论通过小剂量试验扫描可确定最佳扫描延迟时间,常规延迟时间为16~20 s;多种图像后处理技术对颈动脉不同病变的显示各有优势,建议选择应用。
【Abstract】 Objective To investigate the optimal scanning delay time and compare various post processing means in extracranial carotid artery CT angiography.Methods One hundred and fifty-six patients were carried out volume scan by GE Light Speed Plus/4i multiple detector-row spiral CT and divided into two groups.The first group 72 patients’delay time were determined by small contrast dose experimental scanning,the second group 84 patients were routinely delayed 16~20s.All imagings were reconstructed by using MPR,CPR,vessel analysis,SSD,VR,MIP and VE on ADW4.0 workstation.Results Scanning success 152 patients,4 patients were unsucess because 1 couldn’ t cooperate and 1 delayed time too short,2 too long,respectively belonging to the first group and the second group.Lesions include 3 cases carotid body tumors,4 carotid artery aneurysms and 1 pseudoaneurysm.One hundred and seventy-six carotid arterys were no stenosis,72 mild,21 moderate,15 severe and 4 occlusion in the remain 144 patients 288 carotid arteries.Conclusion Small contrast dose experimental scanning can accurately determine the optimal scanning delay time in all patients,16~20s is the routinely delay time.Various post processing means have its advantages in displaying carotid artery diseases.
