内皮素-1、一氧化氮与冠状动脉病变程度相关性研究

【摘要】 目的:旨在探讨冠心病患者内皮素-1(ET-1)、一氧化氮(NO)与冠状动脉病变程度相关性。方法:选择心内科行冠状动脉造影的患者共241例,根据冠脉造影结果分为冠状动脉狭窄组(n=135),非冠状动脉狭窄组(n=106)。比较冠状动脉狭窄组与非冠状动脉狭窄组的ET-1、NO,以及不同病变支数患者ET-1、NO水平与冠状动脉病变程度的相关性。结果:1)冠状动脉狭窄组血浆ET-1高于非狭窄组,而血清NO浓度低于非狭窄组。同时随病变血管增加血浆ET-1水平增加,而NO则相反,差异有统计学意义(P<0.01)。2)Pearson直线相关分析:年龄、糖尿病史、吸烟、TC、LDL-C、ET-1与冠脉病变支数、冠脉狭窄记分呈正相关(r分别为:0.325,0.241,0.227,0.087,0.053,0.552,P<0.01~0.05),NO与冠脉病变支数、冠脉狭窄记分呈负相关(r=-0.504,P<0.05)。TG、BMI、与冠脉病变支数及狭窄记分无相关性。结论:内皮功能障碍是冠状动脉狭窄的重要因素。冠状动脉狭窄程度与内皮素-1呈正相关,与一氧化氮呈负相关。

【Abstract】 Objective:To determine the respective influence of endothelin-1(ET-1) and nitric oxide(NO) on severity of coronary artey stenosis. Methods: 241 subjects underwent coronary artery angiography were divided into two groups according to the results of coronary artery angiography.The plasma level of ET-1 and serum level of NO were compared between the patients with and without coronary artey stenosis.Then the associations between ET-1 ,NO and the severity of coronary artery stenosis were analyzed among the groups. Results:The plasma level of ET-1 and serum level of NO in subjects with coronary stenosis were significantly higher and lower respectively and especially more significant in three-branch lesion patients(P<0.01). Pearson linear correlation analysis showed that the age of the subjects ,the history of diabetes mellitus smoking , the level of TC , LDL-C and ET-1 were positively correlated,while NO was negatively correlated with the number of coronary artery stenosis and the coronary artery stenosis score(r=0.325,0.241,0.227,0.087,0.053,0.552,respectively P<0.01~0.05). Conclusion:Endothelial dysfunction was an important risk factor of atherosclerosis. The severity of coronary artery stenesis was positively correlated with the level of ET-1, and negatively with NO.
