【摘要】 目的观察卡维地洛对老年高血压病人舒张功能不全(diastolicheartfailure,DHF)的临床疗效。方法入选老年高血压病人163例,按纽约心脏协会(NewYorkHeartAssociation,NYHA)心功能分级Ⅱ～Ⅳ级,主要表现为舒张功能不全,分成卡维地洛组85例及对照组78例,两组病人均给予抗高血压及抗心力衰竭(congestiveheartfailure)治疗,但对照组不用β受体阻断药。卡维地洛组病人加用卡维地洛3～25mg/d,疗程6个月。比较两组病人NYHA心功能分级及左心室超声舒张功能指标改善情况。结果①卡维地洛组总有效率为95%(75/85);对照组总有效率为80%(62/78),差异有统计学意义(P<0.05);②治疗前后左心室等容舒张时间(isovolumicrelaxationtime,IVRT);二尖瓣舒张舒张早期和晚期充盈速度比值(E/A)、E峰减速时间(decelerationtime,DT);以及左心房前后径(leftatrialdiameter,LAD),收缩压、舒张压、心率改善明显优于治疗前(P<0.01);卡维地洛组IVRT,E值,E/A,DT,LAD,心率等改善优于对照组(P<0.05)。结论卡维地洛治疗老年高血压病人可显著改善心功能分级和左心室舒张功能。更多还原

【Abstract】 Objectives To investigate the clinical efficacy of carvedilol on the diastolic heart failure in the elderly hypertensive patients. Methods 163 elderly hypertensive patients with diastolic heart failure classified by New York Heart Association (NYHA) classⅡ to Ⅳ were studied,the treated group(n=85) were treated with carvedilol and common anti-hypertensive and anti-heart-failure drugs,and the control group(n=78) were treated with the common drugs alone,then we compared the results after treatments in the treated group with those in the control group. Results All data in the treated group such as isovolumic relaxation time(IVRT),early left ventricular filling velocity(E), Velocity of left ventricular filling contributed by atrial contraction(A),E/A,E-wave decelerate time(DT), left atrial diameter (LAD), systolic blood pressure, diastolic blood pressure, heart rate(HR)etc, were better than those in the control group significantly(P﹤0.05). Conclusions The carvedilol could lower blood pressure,heart rate obviously and improve the heart diastolic function.更多还原
