血液透析与透析滤过对中小分子物质清除的比较

【摘要】 目的:探讨血液透析(HD)与血液透析滤过(HDF)对中小分子物质清除效率的区别,并比较两种方法的心血管稳定性。方法:将22例长期血透患者随机分为两组,分别进行HD和HDF/HD治疗六个月,比较两组治疗前后及两组间血清肌酐、β2微球蛋白浓度的变化,记录两组每次治疗过程中血压、心率、心血管事件等情况。结果:HDF组可以有效的清除β2微球蛋白,治疗前后比较P<0.05,HD组不能达到对β2微球蛋白有效清除,治疗前后比较P>0.05,两组间比较差异有显著性。HD和HDF均能有效清除血肌酐,治疗前后比较P均小于0.05,两组间比较差异无显著性。HDF组低血压的发生率明显低于HD组。结论:HDF能很好的清除中小分子物质,能预防和减少透析相关并发症的发生,并且有很好的心血管稳定性。

【Abstract】 Objective: To compare the difference of hemodialysis(HD) and hemodiafiltration (HDF) in the clearance of the middle and low molecular weight toxins, and in the cardiovascular stability. Methods: In this retrospective study, 22 stable hemodialysis patients underwent HD (10 patients) or HDF/HD (12 patients) for 6 months. The serum creatinine and β2,microglobulin concentration were measured before and after treatments. Blood pressure, heart rate and cardiovascular events were recorded in every treatment course. Results: β2,microglobulin was statistical significantly extracted in HDF group (P < 0.05), and there was no statistical significance in HD group(P > 0.05). Furthermore, significant difference was observed between two groups. An equally significant improvement was observed in regard to the extraction of creatinine in two groups. The incidence of hypotension was significantly lower in HDF group than in HD group. Conclusions: HDF was better than HD in clearance of the middle molecular weight toxins, and also it could decrease the incidence of dialysis,related complications. And HDF had better cardiovascular stability.
