神经端侧吻合对失神经肌肉的营养作用

【摘要】 目的通过对神经端侧吻合的实验研究,了解其对失神经支配的肌肉营养作用。方法12只成年雌性S-D大鼠分成三组。实验组:切断左侧腓总神经,把腓总神经远断端吻合到胫神经的侧方,胫神经外膜开窗,腓总神经近断端结扎反转固定于肌肉上12例。未吻合组:于左侧相应部位切断右侧腓总神经,远、近断端结扎反转固定于附近肌肉上6例。正常组:腓总神经不予切断6例。术后7个月行肌肉组织学检查和神经电生理测试。结果实验侧胫前肌体积和正常组近似,而未吻合组胫前肌萎缩明显。组织学发现实验组胫前肌形态良好,而未吻合组肌肉细胞核明显增多,肌纤维截面积缩小。实验组胫前肌均可引出不同比例的“M”波,波幅接近正常组,而未吻合组却始终未引出诱发电位。结论神经端侧吻合法可以对失神经肌肉提供神经营养作用。

【Abstract】 Objective to study the neurotrophic effect of the end-to-side neurorrhaphy to the denerved muscles.Methods 12 SD rats were divided into three groups: Experimental group,.Experimental group(n=12): Peroneal nerves were transected on the left side of all the animals,the proximal end of peroneal nerve was ligated and sutured reversally to the muscle,the distal end was sutured to the side of windowed tibial nerve.None-neurorrhaphy group: the peroneal nerves were transected on the right side of 6 cases,both ends were ligated and kept apart by suturing them into subjacent muscles.Normal group: the peroneal nerve was left intact on the right side of other 6 cases.7 months after operation,the electrophysiological test was undertaken and the tibial muscle was taken for histologic examination.Results There was significant difference on the volume and histology of both tibial muscles between the experimental group and uncoapted group."M"waves were induced in the experimental tibial muscles with the similar amplitude.While in the none-neurorrhaphy group there were no"M" waves.Conclusion The end-to-side neurorrhaphy can provide the denervate muscles with neuatrophy.
