【摘要】 目的观察化疗加超分割放射治疗Ⅲ期非小细胞肺癌的疗效及毒副反应。方法48例Ⅲ期非小细胞肺癌患者随机分为常规放疗加同步化疗组(常规放化组)25例,超分割放疗加同步化疗组(超分割放化组)23例。两组患者化疗均采用NP方案4个周期。放射治疗均采用8MV-X线照射,常规放化组的放疗剂量为2Gy/次,1次/天,5次/周,总剂量60Gy,6周完成;超分割放化组为1.2Gy/次,2次/天,10次/周,总量69.6Gy,6周完成。结果二组治疗后完全缓解率常规放化组为32.0%,超分割放化组为34.8%;二组近期总有效率常规放化组为80.0%,超分割放化组为82.6%,差别无显著性(χ2=00,P>0.05)。中位生存期常规放化组为18个月,超分割放化组为16个月(t=0.88,P=0.38)。1、2、3年生存率常规放化组分别为72.0%、36.0%、20.0%,超分割放化组分别为69.6%、30.4%、17.4%(χ2=0.41,P=0.52),差异无显著性。毒副反应方面,超分割放化组Ⅲ~Ⅳ级放射性食管炎的发生明显高于常规放化组,差异有显著性(P<0.05)。结论化疗加超分割放射治疗Ⅲ期非小细胞肺癌,生存率未提高,急性放射性食管炎的发生明显增加。更多还原

【Abstract】 Objective:To observe the effect of chemotherapy (CHT) plus conventional radiotherapy (CRT) and chemotherapy (CHT) plus hyperfractionated radiotherapy (HFRT) for stage Ⅲ non small cell lung cancer (NSCLC).Methods:From June 1999 to December 2002, 48 patients with stage Ⅲ NSCLC were randomized into two groups: Arm 1 consisted of concurrent CHT plus CRT(60Gy in 30 fractions during 6 weeks starting on day 1). Arm 2 was concurrent CHT plus HFRT(starting on day 1 with a total dose of 69.6Gy in 58 fractions during 6 weeks, 1.2Gy/fraction b.i.d.). CHT regimen of Arm 1 and Arm 2 was NVB+PDD.Results:The overall response rates (CR+PR) of Arm 1 and Arm 2 were 80.0% and 82.6%, respectively (P>0.05).The median survival time was 18 months in arm 1and 16 months in arm 2. The 1-,2-and 3-year survival rates were 72.0%,36.0%,20.0% and 69.6%,30.4%,17.4%, respectively in arm 1 and arm 2(P>0.05). Severe esophagitis was significantly more frequent in arm 2 than that of arm 1 (P<0.05).Conclusion:Concurrent chemotherapy plus hyperfractionated radiotherapy dose not singnificantly improve the survival rates of stage Ⅲ NSCLC, but with higher acute esophagitis.更多还原
