【摘要】 目的:探讨阿米卡星预防导尿管伴随性尿路感染(UTIC)的临床价值。方法:将102例择期手术前均无UTIC症状的患者,随机分为常规组(52例)和阿米卡星组(50例)。常规组在严格无菌操作留置导尿管。阿米卡星组在严格消毒后,用无菌10ml注射器抽取阿米卡星注射液200g/2L与灭菌石蜡油3.0ml充分混匀后,慢慢注入尿道,再留置导尿管。结果:常规组15例发生UTIC(28.9%),拔管前中段尿培养阳性21例(40.4%),拔管后24h中段尿培养22例阳性(42.3%);导尿管生物膜培养33例阳性(63.5%),共检出14种细菌。阿米卡星组发生UTIC7例(14.0%),拔管前中段尿培养阳性6例(12.0%),拔管后中段尿培养阳性10例(20.0%);导尿管生物膜培养15例阳性(30.0%),共检测出6种细菌。阿米卡星组与常规组比较,发生UTIC明显下降(P<0.05)。结论:阿米卡星预防UTIC疗效确切,操作简易,在临床有实用价值。更多还原

【Abstract】 Objective:Prospective research for the clinical value of Amikacin effect on preventing urethra tract infection from catheter(UTIC).Methods:102 cases was divided into routine group(52cases) and therapy group(50cases) randomly. Catheterization was performed in routine group strictly according to routine protocol. In therapy group, 200mg Amikacin mixed with 3ml aseptic olefin oil was injected into urethra before catheterization.Results:In routine group, irritating symptoms of urethra infection happened in 15cases(28.9), Middle section urine culture was positive in 21 cases(40.4)before withdraw catheter and in 22 cases(42.3) within 24hr after withdraw catheter, Catheter biofilm culture was positive in 33 cases(63.5), 14 kinds bacteria was found in total.In therapy group,irritating symptoms of urethra infection happened in 7 cases(14.0),Middle section urine culture was positive in 6 cases(12.0) before withdraw cathater and in 10 cases(20.0) within 24hr after withdraw catheter, Catheter biofilm culture was positive in 15 cases(30.0), 6 kinds bacteria was found in total. UTIC decreased obviously in therapy group compare with routine group. Statistical treatment shows P<0.05.Conclusions:the Amikacin is effective on preventing UTIC, easy for operating and mastering. The clinical practice value of preventing UTIC by Amikacin is predominant.更多还原
