【摘要】 目的：提高对异位肾上腺组织的认识和诊治能力。方法：报告3例异位肾上腺组织患者的诊治经过,并结合文献进行分析。结果：2例切除结节,病理检查报告为肾上腺皮质组织。1例行淋巴结清除术,病理检查左肾门结节为节细胞神经瘤,腹主动脉旁结节为增生的纤维组织,其内可见肾上腺组织。术后患者均恢复顺利,无肾上腺皮质功能不全的表现。结论：肾上腺组织较常异位至纵膈与盆腔之间的腹膜后任何部位或精索、睾丸旁,常于手术中偶然发现。由于异位肾上腺组织有发生增生或肿瘤的可能性,故一旦发现,应予切除,但术中常规探查是不必要的。更多还原

【Abstract】 Objective To improve the recognition and management of ectopic adrenal tissue.Methods:The clini- cal data of 3 cases of ectopic adrenal tissue were reported and discussed with the related literatures.Case 1 was a boy of 3.5 years old,and a 0.2 cm×0.3 em yellow nodule was found along the right spermatic cord during the op- eration for undescended testis.Case 2 was a 15 years old boy,and a 0.3cm×0.3cm dull yellow nodule was found between the head of the left epididymis and the testis during resection of a cyst of the epididymis and pexia of the left testis.Case 3 was a 50 years old male,several soft nodes 0.3cm to 0.8cm in diameter were found heside the re- nal hilum and the aortic artery during radical nephrectomy for a left renal mass.Results:The nodules of case 1 and cases 2 were resected and ectopic adrenal cortical tissues were diagnosed by pathological examination.The suspi cious lymph nodes of case 3 were resected completely,yet by pathological examination the nodes beside the renal hilum were diagnosed as ganglioneuroma and the nodes beside the aortic artery were diagnosed as hyperplastic fi- brous tissue and normal adrenal tissue.The 3 patients recovered smoothly and no manifestations of adrenal insuffi- eiency were observed after the operation.Conclusions:Ectopic adrenal tissues may be located at any retroperitoneal site from the diaphragm to the pelvis,along the spermatic cord or the testis,and is usually found incidentally dur ing surgical procedures of these sites.As they are associated with hyperplasia and neoplastic change,removal of ectopic adrenal tissue is warranted whenever it is encountered,hut a routine search for them is not mandatory.更多还原
