【摘要】 目的探讨支气管冲洗液菌落计数对兔肺白色假丝酵母菌病的诊断价值。方法新西兰兔36只,随机分为实验组与对照组(各18只),实验组用气管注射法注射0.2ml含1×108白色假丝酵母菌菌液制作肺白色假丝酵母菌病动物模型。对照组同方法注射0.2ml生理盐水。接种后每天按序处死动物,留取肺组织做病理检查;收集支气管冲洗液(BLF)做菌落计数培养;接种前和处死前做肺CT扫描。观察菌落计数与影像显示是否感染及感染轻重的关系。结果BLF菌落计数培养与病理诊断有良好的一致性。若以104CFU/ml为界值,诊断肺白色假丝酵母菌病的敏感性为53.1%,特异性为95.2%;若以103CFU/ml为界值,敏感性为78.1%,特异性为87.5%。BLF菌落计数随影像严重程度增加而增加,χ2=35.58,P<0.005。结论BLF菌落计数培养对兔肺白色假丝酵母菌病诊断与病理有良好的一致性。更多还原

【Abstract】 Objective To assess the diagnostic value of colony count in the culture of bronchial lavage fluid(BLF)for pulmonary candidiasis albicans in rabbits.Methods A New Zealand white rabbits model of pulmonary candidiasis was developed.Thirty-six rabbits were randomly divided into two groups:experimental group and control group.Eighteen rabbits for each group were ranked randomly.The model was formed by per cutem intratracheally injecting 0.2 ml bacterium fluid contained 1×108 candida albicans(C.albicans)in experimental group.The same volume of normal saline was given in control group.The rabbits were serially killed day by day and the pathological changes of lungs were observed.The BLF was collected to culture C.albicans,and the colony was counted.CT scan was performed before injecting and killing for each rabbit in order to analyze the relationship between the counts and X-ray images.Results The colony count of BLF culture displayed a satisfied consistence with pathological diagnosis.Setting 104CFU/ml as critical value the colony count of BLF culture had a diagnostic sensitivity of 53.1% and specificity of 95.2%,whle setting the critical value as 103CFU/ml,the diagnostic sensitivity was 78.1% and specificity was 87.5%.The counts increased as the X-ray images showed deteriorated manifestation,χ2=35.58,P<0.005.Conclusions The colony count of BLF culture had satisfied consistence to the pathology in diagnosing pulmonary candidiasis albicans of rabbits.The critical value of 104CFU/ml should be chosen for correct diagnosis twhile choosing critical value of 103CFU/ml will be helpful to avoid missing diagnosis.The colony counts had an increasing tendency with the deteriorated X-ray images.更多还原
