【摘要】 目的探讨肌松剂在危重哮喘抢救中的应用价值。方法面罩充分给氧同时,异丙酚+维库溴铵诱导下经鼻或经口气管插管,然后用咪唑安定30~120mg/d或异丙酚600~2000mg/d和维库溴铵4~20mg/d微量泵维持,用药量及用药时间视病情而定,配合使用抗生素,静脉应用糖皮质激素,适当水化治疗及静脉营养。结果全部病例在给予肌松剂10~15min后,气道压显著下降,双肺喘鸣音明显减少。经5h~7d机械通气,哮喘完全缓解,治疗前后血气指标显著改善(P<0.01)。结论肌松剂在危重哮喘抢救中有显著疗效,应推广应用。更多还原

【Abstract】 Objective To evaluate the effects of muscle relaxant in urgent treatment of dangerous asthma patients. Methods As facemask oxygen inhaled, endotracheal was intubated from nose/mouth, being induced by propofol injetion and vecuronium bromide injection. It was then maintained by micro-pump of midazolam injection 30/120 mg/d. Depending on patients′ conditions, the medicine was given differently in times and in intervals. Meanwhile, antibiotic, glucocorticoid hormone, hydration and intravenous feedings were used. Results 10 to 15 minutes after using muscle relaxant, the pressure of air passage among all patients dropped obviously and lung wheeze reduced sharply. Through mechanical ventilation ranging between 5 hours and 7 days, the asthmatic symptoms disappeared completely. The measures of blood gas analysis improved significantly after the treatment (P<0.01). Conclusion Muscle relaxant has curative effect in treating dangerous asthma patients. It may be recommended for a wide use.更多还原
