哮喘患者循环型M2受体自身抗体检测及其意义

【摘要】 目的检测哮喘急性发作患者M2受体的自身抗体,探讨M2受体自身抗体在哮喘发病中的作用。方法以细胞外第二环表位肽段的合成肽作为抗原,应用酶联免疫吸附测定技术,检测75例受试者血清中M2受体的自身抗体;哮喘组为40例不同程度的急性发作患者,轻度12例、中度12例、重度16例;正常组为35例正常人作对照。结果①哮喘组M2受体的自身抗体阳性率和抗体滴度为47.5%、1∶75,明显高于正常组的5.7%、1∶20;②重度哮喘组M2受体的自身抗体阳性率和抗体滴度为75.0%、1∶100,明显高于轻度哮喘组(25.0%、1∶40)和中度哮喘组(33.3%、1∶56);③哮喘组M2受体自身抗体阳性组肺功能和临床症状与阴性组比较差异有统计学意义。结论M2受体的自身抗体不仅存在于哮喘患者的血清中,而且抗体滴度也明显高于正常组,提示M2受体自身抗体可能是哮喘发病机制的一个重要环节。

【Abstract】 Objective To investigate whether autoantibodies against M2-muscarinic acetylcholine receptor are related to the pathology in asthma.Methods The synthetic peptide M2-muscarinic acetylcholine receptor which corresponded to amino acids sequences of the second extracellular loop of human was used as an antigen by enzyme linked immunosorbent assay to screen the serum from two groups: asthmatic group(n=40) was divided into three ranks: slight rank(n=12),midrange rank(n=12),heavy rank(n=16) and normal control group(n=35) was collected from normal subjects for health examination.Asthma symptoms and pulmonary function were observed in asthmatic group.Results ①The positive rate of autoantibodies against M2-muscarinic receptor and the antibody titer in the serum from asthmatic group((47.5)%,1∶75) were much higher than those from normal control group((5.7)%,1∶20).②The positive rates of autoantibodies against M2-muscarinic receptor and the antibody titer in the serum from heavy rank((75.0)%,1∶100) were significantly higher than those from slight rank((25.0)%,1∶40) and midrange rank((33.3)%,1∶56).③In comparison between asthma patients with positive and negative serum of autoantibody,asthma patients with positive serum of anti-M2 receptor autoantibodies have shown significant difference with asthma patients negative serum of anti-M2 receptor autoantibodies in pulmonary functions and symptoms of asthma.Conclusion In asthmatic group,the rate of positive antibody and the titers of antibodies to M2-muscarinic receptor in the serum were significant higher than those of normal control group.The asthma group with heavy rank has higher rate of the positive serum and the antibody titer.All these indicate that there was a close relationship between the autoantibodies against M2-muscarinic receptor and the pathogenetic condition of asthma.Thus,it may show antibodies to M2-muscarinic receptor plays a very important role in the development of asthma.
