甲状腺机能亢进突眼征患者眼部超声特征与甲状腺机能的相关性分析

【摘要】 目的通过检测甲状腺机能亢进突眼征治疗前后眼部超声生物学参数、视网膜中央动脉的血流动力学改变及实验室甲状腺功能,以评估甲亢治疗的效果。方法随机选择本院内分泌科根据实用内科学诊断标准确诊而未经治疗的甲状腺机能亢进突眼征81例患者,共162只眼,于治疗前、内分泌科综合治疗≤2年(53例106只眼)及综合治疗>2年(28例56只眼),分别采用彩色多普勒(CDFI)超声仪进行眼部超声生物学测值及视网膜中央动脉血流动力学及甲状腺功能检测,对比分析治疗前后的变化。结果甲状腺机能亢进突眼征综合治疗≤2年,甲状腺功能检测基本正常,眼部超声生物学测值与治疗前对比无明显改变,而视网膜中央动脉血流参数PSV、PMV、EDV、RI、PI、A/D改善非常显著(P<0.01)。综合治疗>2年者,甲状腺功能检测基本正常,眼球前房深度、晶状体厚度、眼轴与治疗前对比无明显变化(P>0.05),球后面积、球尖距与治疗前对比有显著改善,视网膜中央动脉血流参数PSV、PMV、EDV、RI、PI、A/D改善亦非常显著(P<0.01)。结论甲状腺机能亢进突眼征治疗后视网膜中央动脉血流参数及甲状腺功能测值有非常显著改善,其眼部超声生物学测值亦有改善,且随着治疗时间的延长改善越明显。

【Abstract】 Objective To discuss the change of eye’s ultrasonic biological numerical values, CRA blood dynamics and thyroid function indexes in Graves’ disease before and after therapy, and to evaluate the effect of therapy. Methods One hundred and sixty-two eyes of 81 sufferers, who were diagnosed Graves’ disease with exophthalmus without therapy based on practical medicine diagnosis standard, 106 eyes of 53 sufferers, who were treated for two years, and 56 eyes of 28 sufferers , who were treated for over two years were choosed. Their eye’s ultrasonic biological numerical values and CRA blood dynamics were observed by CDFI, their thyroid function indexes were detected, then discussed the changes before and after therapy. Results As for the sufferers who were treated for two years, their lab examination indexes were normal and their eye’s ultrasonic biological numerical values changed inapparently, but CRA blood dynamics PSV,PMV,EDV,RI,PI,A/D ameliorated significantly(P<0.01). as for the sufferers who were treated for over two years, their lab examination indexes were normal, anterior chamber of eyeball depth、lens thickness,optic axis changed inapparently(P>0.05), but retrobulbar area changed evidently (P<0.05), CRA blood stream dynamics PSV,PMV,EDV,RI,PI and A/D ameliorated significantly(P<0.01)too. Conclusion After therapy, the sufferers’ CRA blood stream dynamics and thyroid function lab examination indexes ameliorated significantly, and the eye’s biological numerical values ameliorated too, and which increased more as the time of therapy went on.
