尿微量蛋白质测定方法在筛查早期肾损害中的意义

【摘要】 目的探讨一种简便、快速、准确、能自动化测定尿微量蛋白质(m-TP)的方法,用于筛查肾病、高血压及糖尿病肾损害时蛋白的流失。方法采用邻苯三酚红钼络合(PR)显色法测定尿m-TP,采用苦味酸法测定尿肌酐(CREA),同时对164例正常人和75例肾病、30例高血压、23例糖尿病患者进行检测。结果本实验室尿m-TP参考范围为39.7~135.5mg/L,尿m-TP/CREA值参考范围0.03~0.13。肾脏疾病、高血压肾损害及糖尿病肾损害患者尿m-TP/CREA的比值较正常对照组有明显升高(P<0.01)。结论以PR法取代传统的沉淀法测定随机尿中m-TP,同时测定尿CREA,用m-TP/CREA来评价肾脏病变病、高血压及糖尿病肾损害时蛋白的流失,是临床筛查早期肾脏损伤敏感而快速的实验指标。

【Abstract】 Objective To explore a handy,fast,exact and automatic method to determine the losing protein in the patients with renal disease,hypertension and diabetes mellitus.Methods 75 cases of renal disease,30 cases of hypertension,23 cases of diabetes mellitus and 164 healthy adults as normal contrast were determined for urine microprotein with "PR Method" and urine CREA with“Jaffe Method”.Results The reference range of urine microprotein in our laboratory was 39.7-135.5 mg/L,the reference range of the urine m-TP/CREA was 0.03-0.13.The rate in renal disease,hypertension and diabetes mellitus obviously increased(P<0.0 1).Conclusion Instead of traditional “Deposit Method”,“PR Method” to determine urine microprotein and“Jaffe Method”to determine urine CREA at the same time,the rate of urine m-TP/CREA is a fast and sensitive index to evaluate the losing protein for the patients with renal disease,hypertension and diabetes mellitus.
