【摘要】 目的通过观察脑电双频指数(bispectral index,BIS)在丙泊酚静脉麻醉胃肠镜中的应用,探讨其在指导临床用药、提高麻醉质量及减少麻醉并发症等方面的意义。方法68例拟在丙泊酚静脉麻醉下行胃肠镜患者随机分为两组:脑电双频指数组(BIS组,n=35)和未用脑电双频指数组(对照组,n=33)。记录并比较两组患者在麻醉前、手术中(胃肠镜操作开始后5 min)和手术后的平均动脉压(MAP)、心率(HR)、呼吸(R)、脉搏血氧饱和度(SpO2),平均手术时间,丙泊酚用量,麻醉苏醒时间以及术后麻醉并发症的发生数。结果麻醉期间两组患者均无明显不适感觉,术毕全部患者都顺利唤醒。两组间比较,手术时间无明显差异(P>0.05),患者术前、术中和术后MAP、HR、R、SpO2均无明显差异(P>0.05);BIS组的丙泊酚用量麻醉苏醒时间,麻醉并发症发生率均明显低于对照组(P<0.05)。结论在丙泊酚静脉麻醉胃肠镜中,通过BIS监测麻醉深度下,可避免盲目使用丙泊酚的相对过量,缩短麻醉苏醒时间,显著减少并发症,有一定的临床应用价值。更多还原

【Abstract】 Objective To observe the application of BIS in gastroenterological endoscope with propofol intravenous anesthesia and to investigate the value of BIS on guiding the clinical medication,advancing the quality of anesthesia and reducing the complications of anesthesia.Methods Sixty-eight patients undergoing gastroenterological endoscope with propofol intravenous anesthesia,were randomly divided into 2 groups:BIS group and control group.The MAP,HR,R、SpO₂ before anesthesia,during surgery and after surgery,the average time of surgeries,the propofol dosage,the times of analep and the incidence of complications of anesthesia after surgery were observed and recorded.Results both 2 groups had no significant malaise during the anesthesia,all the patients could be aroused successfully after surgery.Those were no significant differences when compared the 2 groups in ways of the tines of surgeries,MAP,HR,R、SpO₂.Compared with control group,significantly,the propofol dosages of BIS group were smaller,the times of analep were shortened and the incidence of complications of anesthesia was lower.Conclusion Compared with control group,under the inspection of the depth of anesthesia with BIS,the relatively overdo of propofol could be avoided,the times of analep could be shortened and the complications of anesthesia could be significantly reduced.So, the application of BIS is valuable in clinic.更多还原
