激光心肌血运重建术对犬心肌的去神经作用
Transmyocardial laser revascularization denervates canine myocardium
【摘要】 目的:观察激光心肌打孔血运重建(TMLR)术后早期对心肌缺血犬的去交感神经作用。方法:健康杂种犬20只,随机平均分为4组(每组5只),即对照组,切开心包后不作任何处理;心肌缺血组;TMLR组和酚处理组(阳性对照组),于左室相应区域以化学方法去除局部神经结构。术后2周采用W estern印迹、RT-PCR检测各组心肌酪氨酸羟化酶的表达。结果:免疫印迹结果显示TMLR组和酚处理组酪氨酸羟化酶表达阴性。mRNA的表达结果均为阴性。结论:TMLR可破坏犬心脏交感神经及串行于其内的感觉神经,提示临床患者TMLR后症状的改善与痛觉传导障碍有关。
【Abstract】 Objective: To investigate denervation of canine myocardium after transmyocardial laser revascularization(TMLR)by CO2 laser device.Methods: Twenty mongrel dogs were divided randomly into 4 groups(5 dogs each): Ⅰ.sham-operated(negative control)animals undergoing left thoracotomy and pericardiotomy alone;Ⅱ.multiple ligation of the coronary arteries to create acute myocardial ischemia;Ⅲ.40 minutes after creation of acute myocardial ischemia,drilling with high power CO2laser;IV.chemical destruction of cardiac nerves by application of 85% phenol to the epicardium.Two weeks later,cardiac sympathetic afferent nerve function and innervation were assessed by immunoblot(analysis) and reverse transcription polymerase chain reaction(RT-PCR) of tyrosine hydroxylase(TH),a peculiar enzyme of sympathetic nerves.TH mRNA expression was examined by RT-PCR.Results and Conclusion: Immunoblotting analysis and RT-PCR of TH mRNA expression showed negative in laser-and phenol-treated myocardium.Transmyocardial laser treatment may destroy cardiac nerve fibers,including sensory nerve which is contained in sympathetic nerves.Symptom improvement after TMLR could be interpreted by destruction of cardiac nerve fibers conducting angina pectoris in patient.
