影响冻融胚胎移植成功的相关因素分析

【摘要】 [目的]探讨冻融胚胎移植周期的成功因素,以期提高成功率。[方法]对146例共236个冻融胚胎,在自然周期排卵后3~5 d,或未破裂卵泡黄素化综合征(LUFS)周期BBT上升3~5 d时移植胚胎。比较妊娠周期与非妊娠周期的平均年龄、不孕年限、排卵日子宫内膜厚度、移植胚胎数、移植优质胚胎数、排卵后3 d血清E2和P及P/E2值、移植方法以及ET管有无带血。分析胚胎解冻后卵裂球损失的数量及LUFS周期对妊娠的影响。[结果]胚胎质量、ET管血染及移植方法对妊娠结局有显著影响。[结论]在冻融胚胎移植周期,胚胎质量对妊娠的成功率起主要作用。解冻后卵裂球完好的优质胚胎有较好的妊娠结局。使用较软的移植管有助于提高妊娠率。

【Abstract】 [Objective] To evaluate the related influence factors on pregnancy outcome in frozen embryo transfer cycles in order to improve the pregnancy rate.[Methods] Totally 236 frozen embryo transfer cycles were investigated in 146 infertile patients.Embryo transfer was performed during 3 to 5 days after ovulation in nature cycle or 3 to 5 days after basal body temperature(BBT) elevated in luteinized unrupture follicle syndrome(LUFS) cycles.Mean age,duration of infertile,endometrial thickness,the number of embryo transferred,embryo quality,serum progesterone and estrogen levels and their ratio,the method of embryo transfer,embryo transfer(ET) catheter blood staining and blastomere loss on thawing embryos were analyzed.[Results] Embryo quality,ET catheter blood staining and the method of embryo transfer affect the pregnancy outcome significantly in frozen embryo transfer cycles.[Conclusion] Embryo quality plays an important role in the success of frozen embryo transfer cycles.Patients with high quality embryos and intact blastomere may have a good pregnancy outcome.Soft ET catheter is beneficial to the outcome.
