慢性阻塞性肺病继发真菌性医院肺炎的菌群与耐药性分析

【摘要】 【目的】探讨慢性阻塞性肺病(COPD)继发真菌性医院肺炎的菌群与耐药性,以便有效地进行防治。【方法】病例筛选严格参照卫生部《医院感染诊断标准》,真菌鉴定用法国梅里埃VITEK-AMS全自动微生物分析仪及YBC鉴定卡进行。药物敏感试验采用K-B法。【结果】375株酵母样真菌中白色念珠菌居首位,占64.8%;其次为热带念珠菌,占16.8%。5种常用抗真菌药物对真菌显示了较好的抗菌活性,尤以两性霉素B、5-氟尿嘧啶和氟康唑为佳。【结论】临床疑为COPD真菌感染病例须及时送标本培养,以早期发现病原性真菌和选择有效药物治疗。

【Abstract】 【Objective】To study the flora and resistance of chronic obstructive pulmonary disease(COPD) patients with fungus in order to prevent it effectively.【Methods】The cases were selected in accordance with "Diagnosis Standard of Hospital Acquired Infection".The isolates were identified by BioMerieux VITEK Automation Microbe System and YBC identification card.Susceptibility test was performed by K-B disk diffusion method.【Results】Among 375 strains of Yeast-like-fungus,the most organisms were Blastomyces albicans(64.8%);then Candida tropicalis accounted for 16.8%.Five commonly used antifungal drugs showed better antifungus activity,especially amphotericin B,5-fluorouracil and fluconazole.【Conclusion】With regard to the cases of COPD with fungi infection suspected clinically,the samples should be sent for inspection in time in order to discover early nosopoietic fungus and select effective fungicidal drugs.
