非体外循环与常规体外循环冠状动脉旁路移植术围手术期心肌肌钙蛋白I的变化
Perioperative changes of cardiac troponin I in off-pump cornary artery bypass and conventional cornary artery bypass grafting
【摘要】 目的观察非体外循环(OPCAB)与常规体外循环冠状动脉旁路移植术(cCABG)围手术期心肌肌钙蛋白I(cTnI)及肌酸磷酸激酶同功酶(CK-MB)的变化规律,探讨其在心肌损伤诊断方面的价值。方法OPCAB组与cCABG组患者各40例,术前、术后即刻、术后第1d、2d、3d、4d、5d、6d、7d、10d晨采静脉血标本检测血清cTnI、CK-MB。结果OPCAB组与cCABG组术后cTnI、CK-MB均有不同程度升高,然后逐渐下降至正常;cTnI高于正常的持续时间长于CK-MB;OPCAB组较cCABG组cTnI、CK-MB浓度峰值低,恢复正常所需时间短(P<0.05或0.01)。结论OPCAB与cCABG术后cTnI与CK-MB变化规律相似,且cTnI的诊断时间窗宽于CK-MB。
【Abstract】 Aim: To investigate perioperative changes of cardiac troponin I(cTnI),CK-MB and their efficacy in diagnosing myocardial injury in off-pump cornary artery bypass(OPCAB)and conventional cornary artery bypass grafting(cCABG). Methods: A total of 40 cases of OPCABs and 40 cases of cCABGs were collected. Venous blood samples were taken at just before and after operation, the 1st,2nd,3rd,4th,5th,6th,7th,10th d for assessing cTnI and CK-MB.Results: In both groups, cTnI and CK-MB increased at different levels after operation and decreased to normal level, however, the levels of cTnI and CK-MB in OPCAB were lower than those of cCABG. Conclusion: Changes of cTnI and CK-MB are similar after OPCAB and cCABG, and cTnI is of larger diagnostic time window.
