【摘要】 目的探讨DF方案化疗合并后程加速超分割放疗治疗中、晚期食管癌的临床应用价值。方法经病理确诊的中、晚期食管癌90例随机分为两组(单放组和化放组),每组45例;放疗均采用6MVX线外照射,单放组常规分割治疗2Gy/次,5次/周,全疗程总剂量70Gy;化放组放疗前2/3疗程常规放疗2Gy/次,5次/周,共40Gy,后1/3疗程改用加速超分割治疗,1.5Gy/次,2次/天,间隔6h以上,全疗程总剂量70Gy,前程配合DF方案化疗,治疗结束后,按照食管癌诊治规范标准,比较两组疗效及1、3年生存率。结果单放组CR44%,PR51%,1、3年生存率分别为42%、28%;化放组CR51%,PR57%,1、3年生存率分别为74%、53%,经统计学处理两者生存率比较差异有统计学意义(P<0.05),毒副作用两组比较差异无统计学意义(P>0.05)。结论后程加速超分割放疗加DF增敏化疗能明显提高中、晚期食管癌的近期疗效及1、3年生存率,且不增加毒副反应,值得推广应用。更多还原

【Abstract】 Objective To evaluate the effect of late-course hyperfractionated accelerated radiation therapy (LCAHF) plus DF project chemotherapy on the treatment of the intermediate and advanced stage esophageal carcinoma. Methods 90 patients with intermediate or advanced esophageal carcinoma were randomly allocated into two groups (single radiotherapy group and complex group). All patients were given with 6 MV X-ray. Single radiotherapy group received radiotherapy alone was 2Gy perfraction, five times per week, the totle dose waried 70Gy. Totle tumor dose of the other group during the first two-threes of the course was 40Gy, 2Gy perfraction, five times per week , and followed by LCAHF, 1.5Gy, twice daily, more than 6 hours between the twice. Chemotherapy regimen DF project. We studied the efficiency and 1,3 year survival rates between the two groups. Results CR was 44% and PR was 51% in the single radiotherapy group, CR was 51% and PR was 57% in the complex group. The 1 year survival rates in the single radiotherapy group and in complex group were 42% and 74%, the 3 years survival rates were 28% and 53%, there was significant difference between the two groups (P<0.05). The side-effect between the two groups was not significant difference improved. Conclusions LCAHF combined with DF project chemotherapy was effective and well tolerant.更多还原
