经椎弓根椎体植骨治疗胸腰椎压缩骨折
Treatment of Thoracolumbar Compression Fractures Using Vaginal Vertebral Pedicle Pyramid Bone Grafting
【摘要】 目的:探讨经椎弓根椎体内植入颗粒骨治疗胸腰椎骨折的方法和疗效。方法:对67例胸腰椎压缩骨折患者采用经椎弓根椎体内植入颗粒骨,并进行随诊。结果:Cobb角术前平均21.3°(10.8°~31.5°),术后7.4(°3.2°~10.5°)。伤椎前缘高度术前49.15%(21.8~67.9),术后94.8%(91.5~98.7)。一年后94.1%(89.9~97.6)。椎后受阻狭窄程度按W olter分类,术前指数1有17例,指数2有31例,指数3有19例。神经功能按ASIA分级,除2例A级无恢复外,其余均有1~3级恢复。结论:经椎弓根椎体植骨可恢复伤椎椎体高度,重建前中柱稳定性,防止术后椎体高度再丢失和内固定松动断裂。
【Abstract】 Objective: To study methods and effect of granule bone grafting into the vertebral pedicle pyramid for treatment of thoracolumbar fractures. Method: There were 67 cases with thoracolumbar compression fractures were treated with vertebral pedicle pyramid bone grafting and were followed up and analyzed.Result: Cobb’s was preoperatively 21.3°(10.8°~31.5°),postoperatively7.4°(3.2°~10.5°).The front height of the fractured vertebral body was preoperatively 49.15%(21.8~67.9),postoperatively94.8%(91.5~98.7),after one year 94.1%(88.9~97.6).According to the wolter’s classification to evaluate the stenosis of spinal canal,17 cases were index 1,31 cases 2,19 cases 3 preoperatively.20 cases were 0,3 cases 1 postoperatively.According to ASIA,1~3 were recovered of the function of nerves except 2case(A).Conclusion: Granual bone graft into vertebra pyramid in the treament of thoracolumbar compresion fracture can surpport the height of the fracture vertebral body,restore the height of the fractured vertebral pyramid and prohabit in-fiaxtion from fracture.
