【摘要】 目的:探讨在关节镜监护下辅助“C”型臂微创手术治疗胫骨平台骨折的方法和疗效。方法:1999~2003年治疗32例胫骨平台骨折,均为新鲜闭合性骨折,按Schatzker分型Ⅰ型5例,Ⅱ型8例,Ⅲ型14例,Ⅳ型2例,Ⅴ型2例,Ⅵ型1例。合并半月板损伤8例,交叉韧带损伤7例,侧副韧带损伤4例。在关节镜监护下辅助“C”型臂进行小切口复位内固定术,在镜下同时处理关节内合并损伤,观察治疗效果。结果:术后随访8个月~3年,全部患者无切口及关节感染,2例出现皮缘坏死。所有患者骨折均在3~5个月愈合,按相关标准评定疗效,总优良率87.5%。结论:关节镜监护下辅助“C”型臂下微创手术治疗胫骨平台骨折,具有损伤小、复位满意、能同时处理关节内合并伤及术后功能恢复快的优点,值得临床推广应用。更多还原

【Abstract】 Objective:To explore methods and their theapeutic effect in treating tibial plateau fracture under arthroscopy and C-arm fluoroscope.Methods:From 1999 to 2003,32 cases of freshan closed tibial plateau fracture were treated at the hospital.According to Schatzker criteria,5 cases were classified as typeⅠfracture,8 cases as typeⅡ,14 as typeⅢ,2 typeⅣ,2 typeⅤand 1 typeⅥ.Of all the cases in this study,7 cases were omplicated by anterior cruciated ligament injury,4 by collateral ligament injury and 8 by meniscal injury.The fracture were reduced and fixed under arthroscopy and Carm fluoroscope.Meanwhile,intraarticular complications were treated as well.The curative effects were observed.Results:All the cases were followed up for 8 monthes to 3 years.There was no incision and joint infection in this group.Necrosis of wound edge occurred in 2 cases.The fractures healed in 3 to 5 months in all the patients. According to relational criterion,the rate of excellence and fineness was 87.5%.Conclusion:Arthroscopic technique assisted by C-arm fluoroscope in treating tibial plateau fractures had advantages of little injury,accurate reduction,management of associated intraarticular injuries and early recovery.
