新生儿头颅B超回声强度的定量分析与新生儿脑发育的评价

【摘要】 目的探讨新生儿头颅B超回声强度的定量分析与新生儿脑发育评价的关系。方法通过颅脑超声及应用医学图像分析软件对101例不同胎龄新生儿脑灰、白质B超回声强度进行定量分析。结果足月新生儿脑组织不同部位的平均灰度值显示:脑室内的平均灰度值最低为35±4.05,依次递增额叶白质67.60±4.09、枕叶白质78.61±2.95、丘脑基底核84.72±6.35、脉络丛145.85±9.68、颅骨186.67±8.34。比较不同胎龄新生儿颅脑各部位超声背景回声的平均灰度值,其结果显示:丘脑基底核、额叶白质、枕叶白质背景回声的平均灰度值随胎龄增加而增高。本组不同胎龄早产儿与足月儿相比,其各部位超声背景回声的平均灰度值差异有统计学意义(P<0.01)。结论可通过脑回声平均灰度值测定,作为临床评价新生儿脑发育的指标,以减少因人为因素导致分析的主观性。

【Abstract】 Objective To study relation of quantitative analysis in echoencephalography of newborn with evaluation of the brain development in infants.MethodsThe development of cerebral gray、white matters were evaluated in 101 different gestational age infants by echoencehalography and medical image analysis system.Results The average values of gray density in different brain tissues of full term infant: the average values of gray density in ventricles is the lowest(35±4.05),and gradually increase in white matters of frontal lobe(67.60±4.09),occipital lobe(78.61±2.95),thalami(84.72±(6.35)),clump of arteries and veins(145.85±9.68),skull(186.67±8.34).To compare with average values of gray density brain tissues of different gestational age infants that the older is gestational age,the higher is the average values of gray density in whit matters of frontal lobe,occipital lobe,thalami.The average values of gray density in different brain tissues were significantly lower in different gestational age premature infants than in full term infants(P<0.01).Conclusion The brain development in infants can be evaluated with the average values of gray density by echoencephalography and medical image analysis system.
