人源性血小板替代物的成分测定和止血效果观察

【摘要】 目的研究人源性血小板替代物的主要成分和止血效果。方法采用冷冻、融化裂解和干燥技术将来源于人的过期浓缩血小板制备成血小板替代物。应用生物化学方法、流式细胞技术分析其主要成分和检测是否含有ABO血型抗原和HLA-I抗原。采用Russel蝰蛇毒时间测定(RVVT)检测其促凝血活性,并在血小板减少兔体内观测血小板替代物的止血效果。结果血小板替代物主要成分是糖蛋白(GPIb、GPⅡb和GPⅢa)和磷脂组分(磷脂酰胆碱和磷脂酰乙醇胺),不含ABO血型抗原和HLA~I抗原物质;血小板替代物RVVT为14.82s,比正常对照的12.64s延长2.18s;血小板减少兔注射血小板替代物后2、4和6小时,其出血时间比注射前明显缩短。结论人源性血小板替代物主要含糖蛋白和磷脂组分,不含ABO血型抗原和HLA-I抗原物质,其在血小板减少兔体内具有明显的短期止血活性。

【Abstract】 Objective To analyse major components and observe hemostatic activity of platelet substitutes from human platelet derivatives. Methods Platelet substitutes were extracted from human outdated platelets by a frozen-thawed procedure. The major components, such as ABO blood type and HLA calss I antigens of platelets substitutes, were measured by the biochemical method and monoclonal antibody technology. The procoagulant activity of platelet substitutes was detected by the Russell’s viper venom time(RVVT), and the hemostatic activity of platelet substitutes was measured in thrombocytopenic rabbits using platelet substitutes. Results Platelet substitutes were mainly composed of glycoproteins and phospholipids. There were no the ABO blood type antigens and HLA-I antigens in the platelet substitutes. RVVTs in platelet substitutes and the normal controls were 14. 82s and 12. 64s, respectively. The bleeding time of thrombocytopenic rabbits after injecting platelet substitutes at 2, 4, and 6 hours was obviously shorter than that befor injecting platelet substitutes (P < 0. 01). Conclusions The major chemical components of platelet substitutes were glycoproteins and phospholipids. and there were no the ABO blood type antigens and HLA-I antigens in platelets substitutes. Platelets substitutes may play an important role in the primary hemostasis effects on thrombocytopenic rabbits.
