【摘要】 目的探讨BiPAP呼吸机面罩机械通气(FMMV)辅助慢性阻塞性肺疾病(COPD)合并呼吸衰竭机械通气患者撤机的价值。方法将首次撤机失败的28例患者随机分为两组经BiPAP呼吸机面罩通气治疗组(A组)和人工气道机械通气组(B组),比较两组机械通气时间、住ICU时间、医疗费用及病死率等。结果机械通气时间A组(7±3)天,B组(14±3)天;住ICU时间A组(9±3)天,B组(27±6)天。医疗费用A组(13980±2000)元,B组(25000±3213)元;病死率A组14.3%,B组64.3%。两者间比较均有显著差异(P<0.01)。结论对于符合撤除有创机械通气的COPD合并呼吸衰竭而不能成功撤机的患者,应用FMMV,可以避免再插管,减少医疗费用,提高抢救成功率。更多还原

【Abstract】 Objective To explore the value of the BiPAP ventilator face-mask mechanical ventilation (FMMV) assisting the chronic obstructive pulmonary disease (COPD) patients who failed in weaning mechanical ventilation.Methods Twenty-eight patients with COPD and respiratory failure who failed in weaning mechanical ventilation were stochastically divided into two groups:FMMV group (A group) and the artificial machanical ventilattion group (B group),two groups were compared with the time of machinery ventilation,the time of living in the ICU,the medical expense and the case fatality rate and so on.Results The time of machinery ventilation:A group (7±3) days,B group (14±3) days; the time of living in ICU:A group (9±3) days,B group (27±6) days; medical expense:A group (13980±2000) Yuan,B group (25000±3213) Yuan; case fatality rate:A group 14.3% ,B group 64.3% .There is the remarkable difference ( P < 0.01 ) between the two groups.Conclusions The COPD patients company with respiratory failure,which is regarding to the removal of the machine but unable succeed removing it,if used FMMV,May avoid inserting a tube again,reduce medical expense,enhance the rescue success ratio.更多还原
