脊柱重度畸形生物力学矫正术(附20例报告)

【摘要】 目的论述脊柱重度畸形矫正的生物力学机制和介绍M8后路加侧前路手术矫正的方法。方法自1997年9月~2005年9月对20例重度畸形在手术前后摄全脊柱正侧位X线片、Bending片、MRI或CT扫描,术后进行对比分析。结果术前KingⅢCobb角及后凸成角平均108°,术后平均矫正60°。结论根据重度脊柱畸形矫正的生物力学机制而采用的侧前路Ⅴ形减张松解加后路M8器械矫正严重脊柱畸形,对脊柱、脊髓的安全性好,有满意的矫正效果。

【Abstract】 Objective To present the principle of the biomechanics for severe spinal deformity and effectiveness of posterior and lateral anterior M8 instrumentation of clinical application.Methods Patients who underwent operation between September 1997 and September 2005 were evaluated by taking spinal balance X-ray of the all spine,X-ray bending,and MRI.Results The mean preoperative lordosis in the King type Ⅲ was 108 degrees while the postoperative mean angle was 60 degrees.Conclusion According to the principle of the biomechanics for severe spinal deformity,the M8 instrumentation with the posterior and lateral anterior approach can be satisfactory in treatment of severe spinal deformity.
