关节表面骨软骨骨折的MRI表现

【摘要】 目的探讨关节表面骨软骨骨折MRI表现。方法研究骨软骨均有创伤的29例膝、踝部病例的MRI片,FSE、EPI和GRE序列成像,常规矢状位、横断面成像,并分析MRI表现。结果29例共34处骨软骨及软骨下均出现形态和内部信号异常,包括骨软骨同时出现压缩断裂、断裂凹陷及不规则撕脱三种类型。关节表面皮质下骨松质内信号异常。结论关节表面的骨软骨骨折MRI表现可分压缩断裂、断裂凹陷及不规则撕脱三种改变,可同时有皮质下松质骨不同程度的骨挫伤。目的探讨关节表面骨软骨骨折MRI表现。方法研究骨软骨均有创伤的29例膝、踝部病例的MRI片,FSE、EPI和GRE序列成像,常规矢状位、横断面成像,并分析MRI表现。结果29例共34处骨软骨及软骨下均出现形态和内部信号异常,包括骨软骨同时出现压缩断裂、断裂凹陷及不规则撕脱三种类型。关节表面皮质下骨松质内信号异常。结论关节表面的骨软骨骨折MRI表现可分压缩断裂、断裂凹陷及不规则撕脱三种改变,可同时有皮质下松质骨不同程度的骨挫伤。

【Abstract】 Objective To investigate the MRI features of osteochondral fracture in the joint surface.Methods MRI of 29 patients with osteochondral injury in knee and ankle joint was studied. The fast spin echo (FSE), the echo plane image (EPI) and the gradient echo (GRE) sequences were used for investigating the joint surface in 1.5 T MR machine. The sagital and axial plane was used for getting the MRI films. The imaging findings were analyzed.Results The abnormalities of both signal intensity and morphology were found in 34 osteochondral and subchondral sites in 29 patients. Three types of change in morphology comprised depression of cartilage into subchondral bone, osteochondral fracture and indentation, irregular osteochondral avulsion. There were abnormal signals in the cancellous bone under the joint surface. All abnormal MRI signals under the joint surface represented subchondral microfracture and impaction.Conclusion The MRI features of the osteochondral fracture in the joint surface comprise:depression fracture of cartilage and subchondral bone;②osteochondral fracture and indentation;③irregular osteochondral avulsion.Each type of injury is accompanied by subchondral microfracture in different degree.
