MRI对椎体骨折的诊断价值

【摘要】 目的:探讨MRI对椎体压缩性(陈旧与新鲜)和隐匿性骨折的诊断价值。方法:对34例椎体骨折于初诊及伤后3,6,12个月行脊柱MRI检查,分析其信号改变与临床的关系。结果:37个骨折椎体急性期均表现为T1WI低信号、T2WI高信号,伤后3和6个月时分别有7个(18.9%)和22个骨折椎体(59.5%)恢复正常,12个月时全部恢复正常信号。伤后不同时期MRI检查结果相差十分显著。6个椎体陈旧骨折,与正常椎体信号无差别。8个隐匿性骨折椎体形态无异常,MRI T1WI、T2WI示椎体内横行条状低信号,3个月复查50%恢复正常,6个月复查基本恢复正常。结论:MRI能判断骨折修复的不同时期,鉴别椎体陈旧与新鲜骨折,尤其是存在多个椎体变形时则更具诊断价值;MRI检查对隐匿性骨折敏感。

【Abstract】 Objective:To evaluate the role of magnetic resonance imaging (MRI) in the differentiatial diagnosis between fresh and old compression fracture as well as detecting occult fracture of vertebral body.Methods:MRI was performed in 34 patients with vertebral fractures,including acute stage at initial presentation (within 5 hours to 12 days of injury) and following-up (the 3rd month,the 6th month and the 12th month after trauma) respectively.The MR signal intensities and the clinical course of vertebral fracture were correlated and analyzed retrospectively.Results:In the acute stage of injury,all 37 vertebral body fractures were hypointense on T1WI and hyperintense on T2WI.During follow-up,7 vertebral body fractures (18.9%) at the 3rd month,22 fractures (59.5%) at the 6th month and all 37 vertebrae fractures in the 12th month after trauma showed normal signal intensities.Marked significant differences of the MR signal intensities were revealed in different stages of vertebral fractures.There was no signal intensity difference between 6 old fractured vertebrae and intact vertebral body.In the 8 cases of occult fracture,the figure of vertebral body was normal,transverse linear hypo-intensity on T1WI and T2WI was shown,which returned to normal in 4 cases (50%) at the 3rd month and in all 8 cases at the 6th month after trauma respectively.Conclusion:Varied stages of vertebral fracture and its repair can be revealed on MRI,showing significant value in the differentiatial diagnosis between fresh and old fracture of vertebrae,especially in cases with multi-vertebrae compression.MRI is also helpful in detecting occult vertebra fracture.
