血清Leptin、IG F-Ⅰ、IL-2检测在老年骨质疏松症患者的应用探讨
Study on Changes of Serum Leptin,IGF-Ⅰ and IL-2 Levels in Elderly Subjects with Osteoporosis
【摘要】 目的:观察老年骨质疏松症患者血清瘦素(leptin)、胰岛素样生长因子Ⅰ(IGF-Ⅰ)、白细胞介素2(IL-2)及骨密度的变化及其关联。方法:采用放射免疫分析及CHOD-PAP法分别检测75例老年骨质疏松症患者血清leptin、IGF-Ⅰ、IL-2和总胆固醇(TCH)水平,同时行骨密度检测。检测结果与青中年对照组比较。结果:老年女性和男性骨质疏松组leptin水平均较青中年对照组增高(P<0.01),两组患者IGF-Ⅰ和IL-2水平均明显低于青中年对照组,差异有显著性(P<0.05,P<0.01);骨密度值亦明显低于青中年对照组(P<0.01,P<0.05)。相关实验显示,leptin与骨密度呈显著负相关关系,IGF-Ⅰ与骨密度则呈正相关。结论:老年骨质疏松症患者瘦素与细胞因子的代谢紊乱是造成骨量减少和骨形成障碍的重要因素,leptin、IGF-Ⅰ、IL-2和其他与骨代谢有关指标对辅助诊断骨质疏松症具有重要的临床意义。
【Abstract】 Objective To investigate the changes of serum leptin,IGF-Ⅰ and IL-2 levels in elderly subjects with osteoporosis.Methods Serum leptin,IGF-Ⅰ,IL-2 levels(with RIA), total cholesterol contents(with biochemical method) and bone mineral density(BMD) were examined in 65 aged patients with osteoporosis.Results The serum leptin levels in subjects with osteoporosis were significantly higher than controls(P<0.01).The serum IGF-Ⅰ and IL-2 levels in subjects with osteoporosis were significantly lower than those in controls(P<0.05,P<0.01).The BMD was positively correlated with the IGF-Ⅰ levels and negatively correlated with leptin level.Conclusion Derangement of leptin and IGF-Ⅰ,IL-2 metabolism is one of the major causes of decreased bone mass and bone formation elderly subjects with osteoporosis.Measurement of serum levels of these parameters is of diagnostic help.
