ACS患者血浆ADM、ET-1、GMP-140水平变化及临床意义
Clinical Significance of Changes of Plasma ADM, ET-1 and GMP-140 Levels in Patients with Acute Coronary Syndrome (ACS)
【摘要】 目的:研究急性冠脉综合征(ACS)患者血浆肾上腺髓质素(ADM)、内皮素(ET-1)和GMP-140的水平变化,探讨它们在急性冠脉综合征发生发展中的意义。方法:随机选择63例急性冠脉综合征患者作为研究组,并分为两个亚组:不稳定心绞痛(UAP)和无ST段抬高的心肌梗死(NSTEMI)31例为UAP/NSTEMI组;ST段抬高的急性心肌梗死(STEMI)32例为STEMI组。并随机选择稳定型心绞痛(SAP)患者31例为SAP组。33例正常人作为对照组。用放射免疫分析测定ADM、ET-1和GMP-140含量。结果:①血浆ADM、ET-1和GMP-140含量UAP/NSTEMI组、STEMI组均高于对照组(P<0.01)。②对照组与SPA组含量差异无统计学意义(P>0.05)。③SAP组、UAP/NSTEMI组、STEMI组之间含量有显著性差异(P<0.01)。④血浆ADM、ET-1、GMP-140之间呈正相关。结论:①血浆ADM、ET-1和GMP-140之间的相互调节、相互作用在急性冠脉综合征的发生发展中起重要作用。②血浆ADM、ET-1和GMP-140可以反映急性冠脉综合征的严重程度,可作为检测急性冠脉综合征不稳定斑块的生化指标。
【Abstract】 Objective To investigate the clinical significance of changes of plasma levels of adrenomedullin (ADM), ET-1 and α-granule membrane protein (GMP-140) levels in the pathogenesis of acute coronary syndrome (ACS). Methods Plasma ADM, ET-1 and GMP-140 levels were measured with RIA in ① 31 cases of ACS with unstable angina pectoris (UAP) or non-ST segment elevation myocardial infarction (NSTEMI), ② 32 cases of ACS with ST segment elevation myocardial infarction (STEMI), ③ 31 patients with stable angina pectoris (SAP) and, ④ 33 controls. Results The plasma ADM, ET-1 and GMP-140 levels in both patient groups with USP/NSTEMI and STEMI were significantly higher than those in controls (P<0.01). The plasma levels of these substances were not much different between patients with SAP and controls (P>0.05). However, the plasma levels of ADM, ET-1 and GMP-140 in the three groups of patients (2 groups of ACS and 1 group of SAP) were significantly different from each other (P<0.01). Plasma levels of ADM, ET-1 and GMP-140 were matually positively correlated. Conclusion Changes of plasma levels of ADM, ET-1 and GMP-140 might reflect the severity of acute coronary syndrome and might serve as a marker for instability of the atheromatous plaques.
