类风湿关节炎患者滑液中嗜中性核粒细胞巨噬细胞炎性蛋白1α表达水平及意义

【摘要】 目的研究类风湿关节炎(RA)患者滑液(SF)及外周血(PB)中嗜中性核粒细胞巨噬细胞炎性蛋白1α(MIP-1α)的表达水平,探讨其在RA发病中的作用。方法利用梯度密度离心法从RA患者SF及PB和健康对照者PB中分离嗜中性核粒细胞、单个核细胞,应用双抗夹心酶联免疫吸附试验 (ELISA),测定RA患者SF及PB上清中MIP-1α水平,通过反转录聚合酶链反应(RT-PCR)法测定MIP-1α在RA患者SF、PB和健康对照者PB中嗜中性核粒细胞mRNA的表达,并与RA疾病活动性指标和SF中单个核细胞数进行相关分析。结果 RA患者SF中嗜中性核粒细胞MIP-1α水平明显高于PB和健康对照者,且与RA疾病活动性指标和SF中单个核细胞数明显相关。MIP-1α mRNA在RA患者SF嗜中性核粒细胞有明显表达。加入肿瘤坏死因子经24 h培养后RA患者SF嗜中性核粒细胞MIP-1α分泌明显增加。结论 SF嗜中性核粒细胞MIP-1α表达可诱导RA患者关节局部和系统性炎症,作为C-C趋化因子MIP-1α可促进单个核细胞由PB进入关节滑膜及组织,导致滑膜增生、血管翳形成、关节破坏。

【Abstract】 Objective To study the level of macrophage inflammatory protein 1α (MIP-1α) in the synovial fluid and peripheral blood neutrophils in rheumatoid arthritis (RA) and discuss it’s role in RA patho-genesis. Methods Neutrophils and monocytes were isolated from samples of synovial fluid and peripheral blood of RA patients and healthy controls respectively by Ficoll-Hypaque. The MIP-1α level was detected by ELISA and the MIP-1α mRNA was detected by RT-PCR. The relationship between MIP-1α in synovial fluid neutrophils, RA disease activity and monocytes counts in synovial fluid was analysed. Results The MIP-1α level of synovial fluid neutrophils in RA patients was significantly higher than that in the peripheral blood and healthy controls. The level of MIP-1α in synovial fluid neutrophils was correlated with RA disease activity and monocytes counts in synovial fluid. MIP-1α mRNA in synovial fluid neutrophils was overexpressed in RA. MIP-1α level in RA synovial fluid neutrophils was significantly increased after 24 hours incubation with TNF. Conclusion The expression of MIP-1αin synovial fluid neutrophils may induce local and systemic inflammation in RA. MIP-1α, as a C-C chemokine, may promote the infiltration of monocytes from peripheral blood into synovial joints and tissues, leading to synovial proliferation, pannus formation and articular destruction.
