【摘要】 目的探讨妊娠期乳腺癌的诊断、治疗和预后。方法回顾性分析北京大学人民医院1994年~2004年收治的3例妊娠期乳腺癌患者的临床资料。结果妊娠期乳腺癌的发生率是0.02%,平均年龄为30岁。主要临床表现为乳腺肿物,经针吸细胞学或手术活检病理确诊,其中2例患者从出现症状到临床确诊的时间超过9个月。2例患者为Ⅱ期,1例Ⅳ期。Ⅱ期患者放弃胎儿行流产后,行乳腺癌改良根治术,并辅以化疗,分别随访8年6个月和10年8个月;Ⅳ期患者妊娠已足月,行剖宫产后仅切除乳腺肿物,虽辅以化疗,仅生存15个月。结论妊娠期乳腺癌的临床表现和诊断方法与非妊娠期相同,但常延迟诊断。治疗依据肿瘤的临床期别、妊娠周数及患者对胎儿的需求程度。妊娠对乳腺癌的预后无明显影响。更多还原

【Abstract】 Objective To discuss the diagnosis,treatment and prognosis of breast cancer during pregnancy.Methods To review three patients with breast cancer treated in peole’s hospital Peking University from 1994 to 2004.Results The breast cancer is very rare in pregnant patients,the incidence was 0.020%,and the mean age was 30.The main symptoms of the three breast cancer patients was breast mass,the final pathologic diagnose was made through fine needle biopsy or lumpectomy.The diagnosis of two patients was delayed more than 9 months.Two patients were at stage Ⅱ,another was stage Ⅳ.Two breast cancer patients with stage Ⅱaccepted modified radical mastectomy as first line method after abandoning the fetus.And adjuvant chemotherapy was regularly used,they were followed up for 8 year 6 months and 10 year 8months respectively.Another breast cancer patient with stage Ⅳ only undertood lumpectomy after cesarean section,although adjuvant chemotherapy was used she died after 15 months.Conclusions The clinical manifestations and diagnostic procedure in breast cancer patients associated with pregnancy was the same as ono-pregnant patients.But they were usually diagnosed delayed.Treatment principle was according to the tumor stage,gestational age and the desire for fetus.Pregnancy was not a prognostic factor for breast cancer.更多还原
