富血小板血浆复合引导组织再生术在磨牙Ⅱ°根分叉病变治疗的应用
Platelet-rich Plasma Induced with Guided Tissue Regeneration in the Treatment of Mandibular Grade II Molar Furcation Disease
【摘要】 目的比较引导组织再生术(GTR),GTR和牛多孔矿化骨粉(BPBM)联合应用,GTR、BPBM和富血小板血浆(PRP)联合应用3种手术方法对磨牙Ⅱ°根分叉病变的疗效。方法18例患者的18颗Ⅱ°根分叉病变牙,随机均分3组,分别采用GTR、GTR/BPBM及GTR/BPBM/PRP治疗,在术后6,12,18,24个月时观察术牙在探诊深度(PD),临床附着丧失(CAL)和临床附着获得的改变。结果3组患牙术后6,12,18,24个月时,PD和CAL与基线时的PD、CAL比较均有明显改善(P<0.01);而在各观察时间3组间的PD、CAL比较差别无统计学意义(P>0.05)。3组间临床附着获得的比较差别无统计学意义(P>0.05)。结论GTR、GTR/BPBM、GTR/BPBM/PRP均是治疗磨牙Ⅱ°根分叉病变的有效方法;PRP在促进牙周组织再生方面的作用可能更强。
【Abstract】 Objective To evaluate whether Bio-oss in combination with guided tissue regeneration(GTR),a combination of GTR and BPBM(GTR/BPBM),or a combination of platelet-rich plasma,bio-oss and GTR(GTR/BPBM/PRP) can improve the healing of mandibular grade Ⅱ molar furcation as compared with GTR alone.Methods A total of 18 grade Ⅱ mandibular molar furcation defects were divided into 3 groups and were surgically treated with GTR,GTR/BPBM,GTR/BPBM/PRP.Changes in probing depth(PD),clinical attachment level(CAL) were evaluated at baseline,6 months,12months,18months and 24months postoperatively.Results The results showed that 6 months postoperation 3 groups presented with significantly greater PD reduction,and clinical attachment gain than baseline(P<(0.01)).Interestingly,no significant difference was found between GTR alone,GTR/BPBM and GTR/BPBM/PRP(P>0.05).Conclusion This study demonstrate that GTR,GTR/BPBM and GTR/BPBM/PRP can improve the healing of mandibular grade Ⅱ molar furcation defects compared with GTR alone.PRP may be an effective modality of regenerative treatment for class Ⅱ furcation defects.
