不同年龄组急性心肌梗死患者罪犯病变的血管内超声影像特点的初步分析

【摘要】 目的:初步分析不同年龄组急性心肌梗死(AMI)罪犯病变的血管内超声(IVUS)影像特点。方法:74例AMI患者按年龄分为老年(≥65岁)组29例,非老年(<65岁)组45例。应用IVUS对两组的74处病变进行分析。结果:老年组和非老年组的最小管腔横截面积、重构指数、脂池或溃疡面积、脂池或溃疡面积/斑块面积比值分别为(3.6±0.9)mm2 vs (4.2±1.1)mm2、(1.055±0.263)vs(1.188±0.245)、(2.3±0.79)mm2 vs(3.9±0.82)mm2、(21.9±4.2)vs(33.9±5.1),P均<0.05。老年组以纤维钙化斑块为主,非老年组以纤维软斑块为主,两组斑块破裂的发生率分别为51.7%vs 75.6%, P<0.05。结论:AMI的罪犯病变多因不稳定斑块发生破裂所致,但非老年组的病变相对老年组而言更加不稳定。

【Abstract】 Objective:To analysis the relationship between age and the features of the culprit lesion in acute myocardial infarction (AMI) as shown by intravascular ultrasound(IVUS) .Methods: According to age 74 selected patients who underwent IVUS were categorized as Group 1 (age≥65yrs) and Group 2 (age≤65yrs) .Coronary remodeling patterns and plaque morphology of the culprit lesion obtained by IVUS were analyzed. Results:Between the two groups, the area of minimal exterior elastic membranes, the remodeling ratio, lipid - pool or ulcer area, the ratio of lipid - pool or ulcer area to plaque area were(3.6±0.9) mm2 vs (4.2±1.1) mm2, (1.055±0.263) vs(1.188±0.245),(2.3±0.79)mm2 vs (3.9±0.82)mm2,(21.9±4.2) vs (33.9±5.1) ,51.7% vs 75.6% in Group 1 and Group 2, P < 0.05 for all. Conclusion: The rupture of unstable lesion was the crucial reason of AMI. The lesions in non - aged group are more unstable than them in aged group.
