动态血压负荷对高血压患者左室结构及功能的影响
The Impact of Ambulatory Blood Pressure Load on Left Ventricular Geometry and Functions in Essential Hypertension
【摘要】 目的:分析原发性高血压患者不同动态血压负荷(BPL)对左室结构及左室收缩和舒张功能的影响。方法:将106例原发性高血压患者根据动态血压负荷值分为三组,计算左室重量指数(LVMI)、相对室壁厚度(RWT)和左室功能参数。应用单因素方差分析来比较不同组间各参数的差异。结果:LVMI和RWT在动态血压负荷增高组增大,左室收缩功能(mFs代表)与左室舒张功能(E/A比值代表)随动态血压负荷增高而渐次降低。结论:动态血压负荷异常增高加重了左室肥厚和心功能损害程度,40%和70%的动态血压负荷值可作为高血压左室结构和功能的危险分层指标。
【Abstract】 Objective: To evaluate the impact of ambulatory blood pressure iload(BPL) on left ventricular geometry, systolic and diastolic functions in essential hypertensive patients. Method: 106 cases of essential hypertensive patients were divided into three groups according to the values of BPL. The left ventricular mass index (LVMI) ,relative wall thickness(RWT) and the left ventricular function parameters were measured. One-way ANOVA was performed to analyze the differences between groups. Results: LVMI and RWT were increased in higher BPL groups. The left ventricular systolic function (represented by mFs) and diastolic function (represented by E/A) were decreased accompanying the increased BPL. Conclusion:The unusually increased BPL may contribute to the accentuated damage of left ventricular hypertrophy and left ventricular functions in essential hypertension. The BPL value of 40% and 70% were provided the risk factor of the left ventricular geometry and functions.
