bFGF在尿道下裂阴茎下弯组织中的表达及意义

【摘要】 目的探讨尿道下裂阴茎下弯畸形阴茎海绵体腹侧表面组织病理学特点,认识尿道下裂阴茎下弯形成机制。方法研究23例尿道下裂伴阴茎下弯,3例隐匿性阴茎和3例躯体瘢痕病例。采用HE染色及苦味酸-天狼猩红染色检测纤维组织增生改变,免疫组织化学检测组织中碱性成纤维细胞生长因子(bFGF)的阳性表达情况。结果(1)阴茎海绵体腹侧表面组织,在尿道下裂病例存在排列相对整齐的纤维组织;而瘢痕病例的纤维组织排列较紊乱且纤维变性较多;隐匿性阴茎病例则主要由疏松结缔组织组成。与隐匿性阴茎病例对照比较,尿道下裂和瘢痕病例的Ⅰ/Ⅲ型胶原比例明显升高,差异有统计学意义(P<0.05);与尿道下裂阴茎下弯程度比较,差异无统计学意义(P>0.05)。(2)尿道下裂组和瘢痕组bFGF表达强度均较对照组明显升高,差异有统计学意义(P<0.05)。结论尿道下裂并发阴茎下弯者,其阴茎腹侧海绵体表面及尿道板皮下组织可能存在纤维化,且此纤维化与阴茎下弯形成相关。

【Abstract】 Objective To evaluate the pathological characteristics tissues on the surface of cavernosa of ventral penis and to investigate the etiology of congenital ventral curved hypospadias.Methods Tissues on the surface of cavernosa of ventral penis of 23 hypospadiacs with penile curvature were serially sectioned and compared to 3 concealed penis and 3 scar tissues.The selected sections were stained with HE.and Sirius Red.Immunohistochemical staining were with basic fibroblast growth factor(bFGF).Results(1)The hypospadic tissues had amount of relatively organized fibers;scar tissues had deranged and degenerated collagen;concealed penis mainly constructed with loose connective tissue.Compared with concealed penis,the proportion ofⅠ/Ⅲ collagen had increased in the hypospadic and scar specimens(P<0.05).There were no statistically significant among different degrees of curvatures.(2)The expression of bFGF in hypospadic ventral areas were markedly higher than those in concealed penis and scar(P<0.05).Conclusion Fibrous tissues were possibly present in the hypospadic specimens analysed which possibly contributed to penile curvature of hypospadias.
