【摘要】 目的总结十二指肠残端瘘的诊断及治疗经验,以提高对十二指肠残端瘘的诊治水平。方法对1985年10月~2004年10月我院收治的20例Billroth-Ⅱ术式胃大部切除术后并发十二指肠残端瘘患者进行回顾性分析,其中15例采用持续胃肠减压、腹腔通畅引流、TPN、生长抑素、生长激素治疗;5例采用手术治疗。结果21~28d瘘口愈合11例,30~68d瘘口愈合8例,1例因并发上消化道出血、肝肾功能进行性衰竭等全身多器官衰竭(MOF)于瘘后98d死亡。结论持续胃肠减压、腹腔通畅引流、TPN、生长抑素及生长激素治疗,是促进十二指肠残端瘘愈合的重要因素。更多还原

【Abstract】 Objective To summarize the experiences in diagnosis and treatment of duodenal stump leakage and to increase the corresponding level.Mehods A total of 20 cases of duodenal stump leakage after Billroth-Ⅱ gastricresection from Oct.1985 to Oct.2004 were treated with continuous gastrointestinal decompression,abdominal cavity drainage,TPN,somatostatin and somaction.Results Eleven case′s fistula healed after 21-28d,8 case′s fistula were healing after 30-68d.One case was complicated with upper gastrointestinal hemorrhage,liver and kindney function failure and MOF and died after 98d.Conclusion The treatment with continuous gastrointestinal decompression,abdominal cavity drainage,TPN,somatostatin and somacton are the important factors for healing of duodenal stump leakage.更多还原
