【摘要】 目的探讨高血压脑出血(H ICH)术后早期再出血的危险因素。方法回顾分析经手术治疗的124例H ICH的临床资料,分为发生早期再出血组和未发生早期再出血组,124例H ICH中27例发生了早期再出血。分析性别、年龄、术前GCS评分、术前有无瞳孔异常、血肿量、出血部位、手术时机、手术方式、术后血压(收缩压和舒张压)9个因素对早期再出血的影响。结果发生早期再出血组与未发生早期再出血组的术前GCS评分、术前有无瞳孔异常、血肿量、出血部位、手术时机、术后血压(收缩压和舒张压)之间有显著性差异(P<0.05)。仅术前GCS评分和术后血压(收缩压和舒张压)两个因素被引入Logistic回归方程作为有意义的因素(P<0.001)。结论术前GCS评分和术后血压是判断高血压脑出血术后早期再出血的最具参考性的指标。更多还原

【Abstract】 Aim To investigate the risk factors of postoperative early rebleeding in hypertensive intracerebral hemorrhage(HICH).Methods The clinical data of 124 cases of HICH receiving surgical treatment were reviewed.The cases were randomly divided into two groups,the early rebleeding group and the non-rebleeding group.Among all of the cases,there were 27 cases suffering from early rebleeding.Nine factors were analyzed.They were sex,age,preoperative gasgow com scale(GCS) score,preoperative status of pupils,volume of hematoma,hematoma location,operative timing,operative methods,and postoperative blood pressure(systolic and diastolic blood pressure).Results There were statically significant differences between the early rebleeding group and the non-rebleeding in the following aspects:preoperative gasgow com scale(GCS) score,preoperative status of pupils,volume of hematoma,hematoma location,operative timing,and postoperative blood pressure(systolic and diastolic blood pressure)(P< 0.05).Only two facts,preoperative glasgow com scale(GCS) score and postoperative blood pressure,entered the Logistic regression equation as the valid factors influencing prognosis(P<0.001).Conclusion Preoperative glasgow com scale score and postoperative blood pressure are most valuable indices to the early rebleeding.更多还原
