【摘要】 目的探讨大肠腺瘤术后内镜随访检查的临床意义,提高对大肠腺瘤与大肠癌关系的重视。方法总结分析我院外科大肠镜室1984年1月～2004年1月间经内镜下摘除或行手术切除并获得大肠镜随访检查的657例大肠腺瘤患者临床资料。结果腺瘤摘除术后3个月～20年共进行了3258次内镜随访检查,共发现腺瘤376例,总阳性率57.3%(376/657),其中遗漏腺瘤23.7%(89/376),新生腺瘤75%(282/376),复发腺瘤1.3%(5/376);腺瘤伴Ⅱ～Ⅲ级不典型增生65.9%(248/376),腺瘤重度不典型增生局灶癌变2.4%(9/376);腺瘤直径2～30mm不等。结论应进一步提高大肠镜的诊断准确率和手术效果,术后定期内镜随访检查对降低大肠癌的发病率具有重要的临床意义。更多还原

【Abstract】 Objective To discuss clinic significance of postoperative endoscopic checkup in patients with colonic adenoma,and emphasize the importance of relati on ship between colonic cancer and colonic adenoma.Methods The clinic data of postoperative endoscopic followup in 657 cases with colonic adenoma,whom had received resection or endoscopic operation from January 1984 to January 2004 was analyzed.Results 3258 times endoscopic checkups were done from 3 months to 20 years after operation of colonic adenoma,and adenoma was found in 376 cases,the total positive rate was 57.3 percent(376/657),of 376 cases,missed adenoma was 89 cases,newly born adenoma was 282 cases,recurrent adenoma was 5 cases,adenoma had atypical hyperplasia ranged from Ⅱ to Ⅲ class in 248 cases,adenoma had local carcinomatous change or severe atypical hyperplasia in 9 cases,the diameter of adenoma was is from 2 milimeter to 30 milimeter.Conclusion It is important for clinic doctor to improve the diagnostic accurate rate of endoscopic chekup of colon,and realize the relationship of colonic adenoma and conlic cancer eath other,the regular postoperative endoscpic follow is beneficial,it cau reduce the morbidity of colonic cancer.更多还原
