【摘要】 目的　探讨肝脏腺瘤样增生 (AH)的诊断和治疗。方法　对我院近 10年经手术治疗的 14例临床资料进行回顾性分析。均行肝脏手术治疗 ,其中行肝局部切除术 9例 ;同时行断流手术 4例。结果　HBsAg或抗HCVAb阳性 10例。AFP升高3例 ,其中合并肝癌 1例。B超、CT、与MRI检查分别为 14例、6例、与 11例。除 2例合并肝癌以外 ,剩余AH行B超、CT与MRI误诊肝癌及怀疑肝癌分别为 9/ 12、3/ 6与 9/ 9。 14例AH与合并肝癌 3例均经手术和病理确诊。AH与肝癌并存 2例 ,另1例为肝癌切除术后 8个月并发AH。术后合并膈下脓肿 1例 ,围手术期死亡 3例。结论　AH为癌前病变。如不能除外癌变 ,应早期采取积极与适合的手术切除等治疗。更多还原

【Abstract】 Objective To investigate the diagnosis and the therapy of adenomatous hyperplasia(AH)of liver. Methods Retrospective analysis of clinical data of 14 AH cases from our hospital in the past 10 years was undertaken, 14 patients were treated by liver operation,among them 9 patients were performed with partial hepatectomy. At the same time 4 patients received esophagogastric devascularization. Results HBsAg and HCvAb were positive in 8 and 2 patients respectively. Increased serum AFP was proved in 3 patients, one of them suffered from hepatocellular carcinoma(HCC). 14、6 and 11 patients received ultrasonography、CT and MRI, respectively. The result comfirmed that 2 AH cases coexisted with HCC the other 12 AH cases were misdiagnosed as HCC and suspected HCC. All 14 AH cases with 3 complicated HCC were diagnosed correctly by operation and pathology. 2 AH cases coexisted with HCC another AH occurred after HCC resected eight months ago one patient complicated with subdiaphragmatic abscess after operation. Three patients died during operation period. Conclusion AH is a premalignant lesion. If the malignancy can not be excluded early active treatment such as resection should be considered.更多还原
