妊娠肝内胆汁淤积症血清E3、β-hCG与胎盘ER、HCG表达的研究

【摘要】 目的 :探讨妊娠肝内胆汁淤积症 (ICP)患者血清雌三醇 (E3 )及绒毛膜促性腺激素 (β -hCG)水平与胎盘雌激素受体 (ER)、绒毛膜促性腺激素 (HCG)表达强度的关系。方法 :选取 1999年 2月至 1999年 10月在我院住院分娩的ICP患者 2 8例 ,另选同期入院的正常孕妇 2 7例作为对照组。所有孕妇均在分娩前 30分钟内取肘静脉血 3ml,用放射免疫方法测定E3 及 β -hCG。选其中病理组及对照组各 2 0例 ,在分娩后收集胎盘组织 ,用免疫组化方法测定胎盘组织中ER和HCG。结果 :ICP组血清中E3 水平明显高于对照组 (P <0 .0 1) ,β -hCG与对照组无显著差异 (P >0 .0 5 )。在ICP组中 ,胎盘组织ER的阳性率高于对照组 (P <0 .0 5 ) ,而HCG的阳性率在两组间无明显差异 (P >0 .0 5 )。在ICP组中血清E3 水平与胎盘中ER水平间有正相关 (P <0 .0 1) ,而血清 β -hCG与胎盘HCG水平则无显著相关 (P >0 .0 5 )。结论 :雌激素水平升高及胎盘中ER表达增强在ICP的发病中可能起重要作用

【Abstract】 The:To explore the changes of serum Estriol (E 3),β-hCG and expression intensity of Estrogen Receptors (ER), HCG on placenta of intrahepatic cholestasis of pregnancy (ICP). Methods: 28 women with ICP and 27 normal pregnant women were enrolled in the study. All elbow vein blood were drawn within half an hour before deliver.Radioimmunoassay was used to detect the level of free E 3 and β-hCG.Selected 20 cases of ICP group and control group respectively and measured ER and HCG of placenta tissue used immunohistochemical method. Results:(1)The level of E 3 was significantly increased in ICP group than those of normal control gruop (P<0.01) and the level of β-hCG had no difference between two groups. (2)Positive rate of ER on placenta tissue was increased significantly in ICP group than those of control gruop (P<0.05), but positive rate of HCG had no difference between two gropus. (3)There were positive correlation between serum E 3 and ER on placenta in ICP group, and no significant correlation between serum β-hCG and HCG on placenta tissue. Conclusion:There results suggest that the high level of E 3 and the intensive expression of ER on placenta may play an important role in the development of E 3 and the intensive expression of ER on placenta may play an improtant role in the development of ICP.
