【摘要】 目的观察生脉、丹参注射液联合抗生素治疗老年人医院获得性肺炎(HAP)的临床疗效。方法将58例老年人HAP患者随机分为观察组30例及对照组28例,两组均使用抗生素,观察组加用生脉注射液和丹参注射液,疗程均14d;观察治疗前后症状、体征、痰细菌培养检出菌株治疗后转阴情况及T淋巴细胞亚群分布、血液流变学指标的变化。结果治疗后观察组总有效率、痰细菌培养检出菌株治疗后转阴情况均优于对照组(P<0.01或0.05),T淋巴细胞亚群分布及血液流变学指标均明显改善,且优于对照组(P<0.01或0.05);观察组死亡率低于对照组(P<0.05)。结论生脉、丹参注射液联合抗生素能增强老年人HAP患者的细胞免疫功能,改善其血液流变性,提高临床疗效。更多还原

【Abstract】 Objective:To observe the clinical therapeutic effect of Shengmai Injection and Dashen Injection comb ined with antibiotics in treati ng the aged patients with hospital acquired pneu monia(HAP).Methods:58aged patients with HAP were divided randomly into two groups.The controlled group(n=28)was treated with antibiotics,while the treatment group(n=30)was treated with Shengmai Injection and Danshen Inject ion combined with antibiotics.In the selection of antibiotics:mild a nd modera te patients were treated wi th Zi-nacef at a aose of 1.5gram for intravenous dripping,twice a day for 14days.Severe patients were treated w ith Rocephin at a dose of 2.0gram for intravenous dripping,once a day,combined with cin rofloxacin at a dose o f0.2gram for intravenous dripping tw ice a day for 1 4days.In addition,Shengmai Injection100ml and Danshen Injection20ml were given for in-travenous dripping respectively in the treatment group once a day for 14days.Clinical symptoms ,signs ,the changes of focal size,bacterial str ain in sputum,T lymphocytc subsets and hem orrheology were obsevred before and after treatment.Results:(1)After 14days treatment,the total effectiv e rate of the treatment group and the contro lled group were 76.67%and 5 3.57%respectively(P<0.05),mortality rate was 6.67%and 21.4 3%respectivel(P<0.05)and the clearance rate of bacterial s train in spu tum was76.66%and 46.15%respectively(P<0.01).(2)CD 3 ?CD 4 ?and CD 4 /CD 8 of the treatment group increased sig nificantly compared with pretreatm ent (P<0.01),and the increase was more than those of the controlled group(P <0.01),the whole blood viscosity,plasma v iscosity,hematocrit and eryth ro-cyte sedimentation were improved significantly compared with pretreatment,a nd the improvement was better than t hose of the controlled group(P<0.05or P<0.01).Conclusion:Shengmai Injection and Danshen Injection combined wi th antibiotics may enhan ce cell -mediated immunity of the aged patients with H AP,improve abnormal hemorheology,enforce antibacte-rial effect and elevete cl inical therapeutical effect.更多还原
