【摘要】 目的　探索上海地区血清丙氨酸氨基转移酶 (ALT)的检测标准化 ,并进行测定条件下人群ALT参考范围调查。方法　将cfas中含IFCC推荐要求的ALT值传递给一份新鲜混合人血清样品 ,以此作为临时校准品 ,校准后测定不同浓度的血清样品。 19所医院检验科共进行 7次实验 ,并在测定条件下检测 2 0岁以下到 71岁以上 75 98人的ALT。结果　ALT在 3 0 0U /L以下时 ,各参加医院检验科之间的平均测定值具有良好的可比性 ,CV <5 .0 % (1.80 %～ 4.60 % )。两次实验之间测定值的CV <3 .4% (3 .3 4%～ 3 .3 9% )。 75 98例ALT测定值男性或女性的第 97.5百分位点分别为 66U /L和 5 5U/L ,约相当于 3 5或 2 9赖氏法测定单位 ;第 99百分位点分别为 74U/L和 64U/L ,约相当于 3 9或 3 4赖氏单位。结论　采用统一校准血清 ,可使不同单位 (不同方法 )间ALT测定值差异明显减小 ,可比性显著增加更多还原

【Abstract】 Objective To examine the possibility of harmonization of ALT results from different laboratories in Shanghai area and investigate the reference range. Methods A pooled patients’ serum, giving IFCC’s ALT level from cfas,was used as caliberator and sent to 19 laboratories together with 2 to 5 fresh patients’ serum samples and tested for 7 times. The reference ranges in 4000 males and 3598 females,ages <20 to >70 years,were determined with standardized method.Results Before harmonization,the interlaboratory CVs varied from 17.1% to 20.5%,and after harmonization,CVs decreased to between 1.8% to 4.6%. The average run CVs were 3.34% to 3.39%. The reference range with standardized method in the 99th pecentile for men and women was 74U/L or 64U/L,respectively. Conclusion The pooled patients’ serum could be used as caliberator for harmonization of ALT results among the laboratories.更多还原
