猪原位肝移植术的麻醉探讨
Discussion of the anesthesia of pig orthotopic liver transplantation surgery
【摘要】 目的:探讨动物猪肝移植模型的麻醉处理和血流动力学的变化旨在为临床肝移植提供资料和经验。方法:头健24康杂种幼猪随机分成两组,供体头,受体头,气管插管静吸复合全身麻醉下实施手术,监测、、的变化,1212MAPCVPACT无肝期施行,观察三个时期的血流动力学变化。VVB结果:猪因其解剖的特殊性,行气管插管较困难,技术要求高,本组经口气管插管比较顺利,例经口插管成功,例行气管切开;无肝期和新肝早期的血流动力学变化最显著,体外静脉-静脉转213流有效地防止了下腔静脉阻断后血液回流障碍,较好地维持了循环的稳定;无肝期、新肝早期、新肝期体温下降和的改ACT变较无肝前期有显著性差异。结论:无肝期使用有利于维持血流动力学和生化的稳定,为临床肝移植提供了宝贵的经EVVB验。
【Abstract】 Objective: To get the experiment data and experiences for clinical liver transplantation, the anesthesia method and hemodynamic changes were observed during the surgery of pig liver transplantation model. Methods: 24 healthy, young and cross-racial pigs were divided into two groups at random. 12 donators, 12 receivers. During balanced-anesthesia, the changes of MAP, CVP, ACT were monitored. VVB was done at anhepatic phase. The hemodynamic changes during 3 phases were monitored. Results: Because of the special anatomy of pigs, the intubation was difficult and requires higher techniques. The oral intubation was successful in 21 cases, and tracheotomy in 3 cases. The hemodynamic changes were remarkable during anhepatic phase and primary neohepatic phase . The blood back flow blockage was successfully prevented by extracorporeal vein-vein bypass, and the stability of circulation was well maintained. Being compared with the pre-anhepatic phase, there were remarkable differences of the dropping of shell temperature and the changing of ACT during anhepatic phase, primary neohepatic phase and neohepatic phase. Conclusion: Using EVVB during the anhepatic phase can be helpful to maintain hemodynamics and blood biochemistry stable. It is an important experience for clinical liver transplantation.
