【摘要】 目的 :探讨内固定在脊柱结核治疗中的作用。方法 :1991年 5月～ 2 0 0 1年 2月共开展脊柱结核内固定手术 2 2例 (其中 1例先做头颅骨盆环牵引、后做内固定手术 )。使用的固定器械 :鲁氏棒 +椎板下钢丝 6例 ,Dick椎弓螺钉系统 8例 ,Harringtonrod 5例 ,SDRS 2例 ,USS 1例。结果 :本组 2 2例脊柱结核内固定手术均未引起炎症扩散。在脊柱结核一期病灶清除、同时或二期内固定 12例中卧床时间比无内固定手术缩短 4～ 8周。结论 :在脊柱结核治疗中内固定手术可以加强脊柱稳定、缩短卧床时间、预防或矫正畸形。但是 ,脊柱结核必定是一种特异性感染 ,选择适应证时还要慎重 ,有些细节问题还要进一步探讨。更多还原

【Abstract】 Objective:To evaluate the role of the internal fixation in the surgical treatment of spinal tuberculosis.Methods:Since May 1991 to Feb.2001,22 patients with spinal tuberculosis were treated by internal fixation.The surgical procedures performed in this study included Luque instrumentation in 6 patients,Dick pedicle screw system in 4,Harrington instrumentation in 5,SDRS in 2 and USS in 1.Out of the 22 patients,one underwent two stage procedure,i.e.Hall pelvic traction followed by internal fixation.Results:Neither the local nor the systemic spread of the tuberculosis was found in all the 22 patients.In 12 patients who underwent the debridement with one staged or two staged internal fixation,the time of bedrest was 4～8 weeks shorter than that of the patients without internal fixation.Conclusion:The internal fixation may play the following roles in the treatment of spinal tuberculosis:to stabilize involved spinal segments and to prevent and correct local deformities with less bedrest time.However,it should be cautious in the selection of indications with respect to the nature of specific infection of tuberculosis.Some technical details still require further evaluation.更多还原
