【摘要】 目的　观察围术期输血对肺癌病人CD3、CD4 、CD8、CD4 /CD8的影响 ,对输血与机体免疫抑制的关系作一前瞻性的研究。方法　对 40例肺部肿瘤的病人随机分组 ,输血组病人在术中输血 6 0 0ml,测定、比较输血组与非输血组手术前后外周血T细胞亚群的变化。结果　肺癌病人在术前的CD3、CD4 、CD4 /CD8明显低于良性肿瘤组 (P <0 0 5 ,P <0 0 0 1,P <0 0 0 1) ,输血组在术后表现为CD3、CD4 /CD8的降低和CD8的明显升高 (P <0 0 5 ,P <0 0 0 1,P <0 0 0 5 ) ,与非输血组相比较差异均有显著意义 (P <0 0 1,P <0 0 0 1,P <0 0 0 1)。结论　肺癌病人的机体免疫功能处于抑制状态 ,T细胞亚群的测定能够反映这一状态的存在 ;围术期输血对肺癌病人的免疫功能有抑制作用。更多还原

【Abstract】 Objective To explore the effect of perioperative blood transfusion on T-cell subsets and relation ship between perioperative blood transfusion and immunosuppression in patients with lung cancer.Methods Fourty patients with lung tumors were devided at random into groups.Fifteen cases received 600ml blood transfusion.The changes of T-cell subsets were detected and compared between the patients with and without transfusion.Results A decreased preoperative T-cell subsets were found in the patients with lung cancer,and the ratio of the CD 4/CD 8 in clinical stage Ⅲ was obviously lower than those of stage Ⅰ～Ⅱ.After operation,the level of T-cell subsets decreased obviously in the transfused group compared with the untransfused one,CD 3 and the ratio of CD 4/CD 8 was significantly lower(P<0.01,P<0.001),the level of CD 8 was obviously higher(P<0.001).Conclusion The patients with lung cancer show a decreased immune state,and perioperative blood transfusion can result in further immunosuppression.更多还原
