【摘要】 目的　探讨经尿道前列腺电汽化术 (TUVP)后阴茎勃起功能的影响因素。　方法　通过问卷调查方式 ,用IIEF 5对 86例TUVP患者的阴茎勃起功能进行问卷评分 ,并将结果与患者年龄、前列腺大小、手术时间、术中出血量、包膜损伤、术后感染、逆行射精及术后IPSS进行相关性分析。　结果　TUVP术后 1年的IPSS由术前平均 2 1.8分降至 6 .8分 ;IIEF 5评分由术前平均 15 .4分降至 10 .0分 ,手术前后比较差别均有显著性意义 (P <0 .0 5 ) ,勃起功能情况与前列腺大小、手术时间、术中出血量、术后感染及排尿症状改善无关 ,与年龄 (r =- 0 .5 311,P =0 .0 0 36 )、包膜损伤 (11/6 1,r= - 0 .4 5 71,P =0 .0 14 5 )及逆行射精有显著相关性 (31/6 1,r=- 0 .4 178,P =0 .0 2 6 9)。　结论　尽管年龄是导致术后勃起功能下降的主要因素 ,但避免TUVP手术并发症和做好相关知识的科普工作能减少手术后勃起功能障碍的发生。更多还原

【Abstract】 Objective To investigate the influencing factors of penile erectile function following transurethral electrovaporization of the prostate(TUVP). Methods Using the international index of erectile function 5 (IIEF 5) questionaire to survey penile erectile function in patients undergoing TUVP preoperatively and postoperatively.The associations of the postoperative IIEF 5 results with patient age,prostatic volume,operative time,amount of bleeding during operation,trauma of the capsule,urinary infection,retrograde ejaculation,international prostate symptom score(IPSS) was analyzed. Results The questionnaire result of postoperative IIEF 5 was significantly lower than preoperative one year ( P <0.05).The erectile dysfunction postoperative was poorly correlated with prostatic volume,operative time,bleeding during operation,urinary infection and IPSS,but strongly correlated with age( r =-0.5311, P =0.0036), trauma of the capsule(11/61, r =-0.4571, P =0.0145), retrograde ejaculation(31/61, r =-0.4178, P =0.0269 ). Conclusions Although age is the major factor in causing postoperative erectile dysfunction,incident of erectile dysfunction can be reduced by avoiding complication of operation and preoperative education about knowledge of the surgery and its outcome.更多还原
