【摘要】 目的　探讨国产及进口乙型肝炎免疫球蛋白 (HBIG)阻断HBeAg/或抗HBe( + )、HBV DNA定量PCR( + )孕妇所生的新生儿宫内乙型肝炎病毒感染的效果。方法　于孕期第 6个月起每月给予孕妇进口或国产HBIG 2 0 0IU肌注 1次 ,预产期前 5～ 7d再加强注射 1次 ,新生儿娩出后 3h内及第 15天各肌注 1次HBIG ,以后按 1、2、7、13个月注射基因工程乙肝疫苗 10 μg。 结果　生后 13个月时小儿HBsAg阳性率 :国产HBIG组、进口HBIGBayer组与Berna组分别为 2 7%、2 2 %、2 8% ;抗 HBs阳性率分别为 94 7%、95 6%、97 2 %。 13个月龄儿国产HBIG组 3例、Bayer组和Berna组各 1例检获HBsAg( + ) ,乙型肝炎病毒母婴传播阻断率分别为 96 9%、97 4%、96 7%。结论　国产HBIG阻断HBeAg( + )孕妇新生儿宫内感染与进口HBIG一样有效 ,且国产HBIG安全、价廉更多还原

【Abstract】 Objective To study the efficacy and safety of HBIG made at home and abroad in blocking the intrauterine hepatitis B virus (HBV) infection in pregnant women who were seropositive for HBeAg or anti-HBe and positive for HBV-DNA by quantitative PCR. Method Intramuscular injection of home-made HBIG 200 IU was given to 115 pregnant women per month from the 6 th month of pregnancy, and one injection was given at 5-7 th day before the expected date of confinement (EDC). Two imported HBIG preparations (Bayer and Berna) were given to 45 and 36 pregnant women respectively at the same dose and schedule. After labor, the newborns were given im HBIG both within 3 hours and at the 15th day. Later, at the 1st, 2nd, 7th and 13th month, the infant were injected with 10 μg of hepatitis B vaccine, respectively. Result The HBsAg positive rates of the newborns of home-made HBIG group, Bayer group and Berna group were 2.7%, 2.2% and 2.8%, respectively. The anti-HBs positive rate was 94.7%, 95.6% and 97.2%. When the babies were at the age of 13 months, 3, 1 and 1 in the 3 groups were confirmed to be HBsAg positive. The blocking rates were 96.9%, 97.4% and 96.7%, respectively. Conclusion The home-made HBIG was as effective as the imported ones in blocking the intrauterine HBV infection in HBeAg positive pregnant women and was also safe and inexpensive.更多还原
