肾盂赘生物的腔内导管超声诊断初步研究
Preliminary study on endoluminal sonography of renal pelvic neoplasms with miniature atheter-based probe
【摘要】 目的 研究肾盂赘生物腔内导管超声的检测特性、声像图特征及其病理改变。方法 使用腔内微探头导管超声探测 10例肾盂赘生物 ,并和手术病理结果比较。结果 10例超声诊断肾盂赘生物均为手术证实 ,其中肾盂移行上皮癌 7例 ,肉芽性病变、肾盂息肉、肾癌侵犯肾盂各 1例。赘生物位于肾盂上部 4例 ,肾盂下部和漏斗部各 3例。超声检出的最小赘生物为 5mm× 2mm。肾盂腔内型赘生物 9例 ,腔外型 1例。赘生物呈多形性改变 ,表现为肿块型、乳头型、地毯型、窄条型、环型及肾盂壁局部缺失型。多数赘生物呈中等或低回声。结论肾盂内导管超声可精确显示肾盂赘生物的形态、大小、回声、部位和来源 ,可检出较小的肾盂内病灶 ,可初步估计其病理性质。
【Abstract】 Objective To investigate ultrasound pattern of renal pelvic neoplasms,their pathologic changes and detective efficacy of endoluminal ultrasonography.Methods Ten cases of renal pelvic neoplasms with painless haematuria (9 cases) and renal colic associated with haematuria (1 case) were diagnosed by ultrasound,using a 10 MHz miniature ultrasound probe with its outer catheter.The probe was inserted into the ureter detected through a cystoscope and advanced up to the renal pelvis.The ultrasound findings were compared with surgery and pathology.Results Of 10 cases,4 tumors located at the upper part of the renal pelvis,3 at the funnel part of the pelvis,another 3 at the lower part of the renal pelvis.The smallest lesion dedected by endoluminal ultrasonography is 5 mm×2 mm in size. Pathologic findings of these tumors were transional cell carcinoma in 7 cases,granuloma in one case,renal polyp in one case,renal cell carcinoma with infiltration of the pelvic wall in one case.Their sonograms appeared as various patterns:papillary shape,carpet like,strip shape,ring and local defect of the pelvic wall.Majority of neoplasms appeared as a hypoechoic or isoechoic.Conclusions Endoluminal ultrasonograghy provided information about the shape,size,echogenicity and location for diagnosis of renal pelvic neoplasms.Tumor as small as 5 mm can be detected by endoluminal sonography.
