【摘要】 目的:了解15mm以下和16～30mm两组周围型肺癌的临床表现以及影像学表现的差别。方法:回顾性分析经手术和病理证实的15mm以下周围型小肺癌59例和16～30mm周围型肺癌85例的临床表现以及胸部平片和CT表现。结果:1)15mm以下周围型小肺癌59例中,病理Ⅰ期53例,Ⅱ期以上6例。16～30mm周围型肺癌85例中,病理Ⅰ期19例,Ⅱ期以上66例。2)胸部平片对15mm以下周围型小肺癌的检出率为52.5%,漏诊率为47.5%;对16～30mm周围型肺癌的漏诊率为2.4%。16～30mm周围型肺癌的毛刺、分叶和胸膜凹陷征等明显高于15mm以下组,而肿瘤密度中的含气型和中间型、支气管气相和支气管血管集束征的出现率,则以15mm以下组为多。结论:1)15mm以下周围型小肺癌的绝大多数无临床症状并且为病理Ⅰ期,其影像学表现和诊断具有重要的临床意义。2)15mm以下周围型小肺癌的影像学表现和检出率与16～30mm周围型肺癌比较有明显差别。更多还原

【Abstract】 Objective:To study the differences of clinical aspects and imaging findings be-tween≤15mm and16～30mm peripheral lung cancer.Methods:The clinical aspects and chest film,CT findings of≤15mm and16-30mm peripheral lung cancers were compared and analysed retro spec tively.Results:1)Of 59cases of small peripheral lung cancer(less than15mm in di -ame ter),5cases were diagnosed from symptoms such as cough,hemoptosis etc,37cases were asymp tomatic and detected by mass screening,17cases were diagnosed for reasons unrelated to lun g cancer.In this group,there were53cases of stage I cancer pathologically.In85cases of16～30mm peripheral lung cancers,47cases were symptomatic(productive cough),25cases were diagnosed for other reasons.13asymptomatic cases were detected by mass screening.There were19cases of stage I in this group.2)47.5%of the small peripheral lung cancers less than15mm in diameter were overlooked at chest film,and2.4%of16-30mm peripheral lung cancer were overlooked at chest film as well.Spiculate,lobulated tumors and chest excava tion were more often seen in16～30mm than in≤15mm peripheral lung cancers,but low density and air-bron chogram were more often found in≤15mm peripheral lung cancers.Con clusion:1)It showed that vast majority of≤15mm peripheral lung cancers are stage I and asymptomatic,and it is of great clin-ical signifi cance to be diagnosed by imaging detection.2)The chest film and CT features of small peripheral lung can cer(less than15mm in diameter)are different from those of16-30mm lung cancer,and CT guided biopsy is recommended.更多还原
